Home Health Physical Therapist (PT)

Full Time/ Monday through Friday 8a-5p / Harker Heights Territory

Your Impact

You’ll help patients improve strength, mobility, balance, and independence by:

- Evaluating functional needs and creating individualized treatment plans
- Providing skilled physical therapy in the home
- Tracking progress and adjusting care as needs change
- Teaching patients and caregivers strategies to support recovery between visits
- Coordinating care with physicians and other healthcare professionals
- Completing timely, accurate clinical documentation
- Supervising therapy assistants and support team members as appropriate

What You’ll Need

- Graduation from an APTA-approved physical therapy program
- A current, unrestricted Texas PT license
- At least one year of experience in a healthcare facility
- A valid driver’s license and acceptable driving record, or reliable transportation between patient homes and work locations
- Ability to frequently lift, push, pull, and support up to 50 pounds, including patient positioning, transfers, and moving equipment
