As a Home Health RN, you'll provide skilled nursing care, support patients and families through health challenges, and help individuals live safely and independently at home. Every visit is an opportunity to make a lasting impact.
What You'll Do
- Provide high-quality nursing care in patients' homes
- Assess patient needs and develop individualized care plans
- Educate patients and caregivers on medications, treatments, and disease management
- Collaborate with physicians and interdisciplinary team members to coordinate care
- Maintain accurate and timely clinical documentation
What We're Looking For
- Current RN license in Maine
- Valid driver's license and reliable transportation
- Strong clinical assessment and communication skills
- Home health experience preferred, but not required
