About this role

About Correctional Healthcare Partners:

Correctional Healthcare Partners (CHP Health) is a company dedicated to providing healthcare services to the incarcerated, and we're growing. We exist because the people inside county jails still deserve real healthcare — and the counties that serve them deserve a partner who shows up, owns the outcome, and gets the work right. We deliver medical, mental health, and dental care across jails and detention facilities in California and Washington, and we do it with clinicians who chose this setting and a leadership team that supports the mission.

J ob Description :

As a Rapid Response Provider, you are the first clinical safety net for patients at CHP correctional facilities on your assigned days. You connect with on-site nursing staff via telehealth (laptop or tablet) to assess, treat, and stabilize patients in real time. On your assigned days, you may also be given scheduled telehealth work to complete between rapid response calls, and during quieter periods you will work through documentation in the EHR while remaining available for urgent requests at all times.

Position Requirements :

Active, unrestricted Nurse Practitioner (NP) license in the state(s) where you will be working. Current DEA registration. Valid CPR/BLS certification. At least 1 year of experience in primary care, urgent care, emergency medicine, or correctional healthcare is preferred. Strong communication and computer skills required.

Location: San Diego Adult Detention Facility

Schedule: Your rapid response days are set in advance. You are available for the full 24 hours on those days, which may include a mix of scheduled telehealth visits, documentation, and rapid response calls as needed.

Compensation: $90-105 /hr

Classification : Non-Exempt

What You'll Do:

- You are the sole rapid response provider during your assigned 24-hour periods — your supervisor covers the days not assigned to you

- Maintain availability via Microsoft Teams during your full assigned shift

- Nursing staff will contact you via Teams message first; if you do not respond within a reasonable time based on patient acuity and nursing judgment, they will follow up with a direct call

- Emergencies and urgent matters require prompt response — routine requests like refills can be handled within a reasonable window at your clinical discretion

- During daytime hours, you may be completing scheduled telehealth visits or documentation between rapid response calls — you remain available for urgent requests throughout

Stabilize Patients Within 24 Hours of Intake

- Assess newly incarcerated patients and initiate a stabilization plan within the first 24 hours of arrival

- Prescribe medications necessary for medical and psychiatric stabilization, including mental health medications, within the initial 24-hour window

- Provide immediate mental health support for patients presenting with acute psychiatric or behavioral health needs at intake

- Coordinate directly with on-site nursing staff and mental health providers to ensure handoff and follow-up care is in place beyond the stabilization period

- Document all intake assessments, stabilization plans, and medication orders completely and accurately in the EHR

See Patients via Telehealth

- Conduct video visits with patients at assigned facilities via laptop or tablet — a nurse on-site supports the encounter

- Evaluate patients with a range of conditions, from routine sick visits to urgent and emergent presentations

- Prescribe medications, order labs, and put in referrals as needed

- Coordinate with the nursing team to make sure your plan gets carried out safely

- Complete all clinical approvals within 7 days of the visit

Handle All Your Own Documentation

- Complete SOAP/SOAPE notes, intake assessments, chronic care documentation, and progress notes for every encounter

- Enter all orders — medications, labs, referrals — into the EHR yourself

- Use available downtime during your shift to work through documentation and orders in the EHR

- Make sure your charting is accurate, complete, and done during or shortly after your shift

Review Labs, Diagnostics, and Return-from-Offsite Patients

The following tasks are for your assigned telehealth days — not rapid response hours, unless nursing flags something urgent:

- Review and sign off on pending lab and diagnostic results

- Review patients returning from outside care (hospital, specialty, ER) and follow up on any new diagnoses or treatment recommendations by the next business day

- These tasks will be assigned to you as tasks within the EHR so you can work through them efficiently during your shift

Support Quality and Continuity of Care

- Participate in chronic care management and follow established clinical guidelines

- Flag anything that could impact patient safety or contract performance to the Medical Director

- Strive to prescribe within formulary whenever clinically appropriate

Work Well with the Team

- Build solid working relationships with nursing staff, site supervisors, mental health providers, and facility security personnel

- Communicate clearly and professionally — written and verbal

- Share important updates with the right people when something needs attention

- Participate in meetings, trainings, or quality improvement activities when requested

Outside of your regular rapid response days, you may be asked to pick up supplemental work, including:

- In-person coverage at a facility

- Covering an additional rapid response day if someone is out
