Description

Director of Hospice Intake & Admissions

##

Lead the way patients and families get to hospice care

When a family needs hospice, every hour counts. At LifeCare, we believe every appropriate referral should get prompt attention, and every patient and family should have a compassionate, well-coordinated experience. We’re looking for an experienced RN leader to build and run the centralized intake and admissions operation that makes that happen across our hospice agencies in several states.

As Director of Hospice Intake & Admissions , you’ll report to the Vice President of Sales. You’ll connect sales, clinical operations, referral partners, and admission teams so that each referral moves to admission quickly, compliantly, and with the patient at the center.

###

What You’ll Do

Build and lead a high-performing intake operation

- Oversee all hospice intake and referral coordination across your assigned agencies and markets.
- Create standard, centralized workflows for receiving referrals, clinical review, insurance verification, physician coordination, consents, and admission scheduling.
- Remove barriers that slow down admissions, and keep coverage in place, including after hours and on weekends.

Be the clinical and compliance expert

- Guide intake staff on hospice eligibility, Medicare LCDs, and admission requirements.
- Keep workflows in line with Medicare Conditions of Participation, CMS, HIPAA, and state regulations.
- Work with medical directors, attending physicians, and clinical leaders on certifications and eligibility decisions.

Improve referral conversion

- Set up real-time referral tracking from the moment a referral arrives until it’s resolved.
- Watch conversion rates, response times, turnaround, and the reasons referrals don’t lead to admission.
- Partner with sales leadership and hospice liaisons to keep referral sources informed and to solve problems quickly.

Grow with us

- Design and refine a scalable centralized intake model that serves multiple agencies and states.
- Look for ways to automate and integrate technology, working with IT, compliance, billing, and clinical operations.
- Bring newly acquired agencies onto the same intake processes as LifeCare grows.

Develop your team

- Recruit, train, mentor, and evaluate intake staff.
- Set clear expectations and productivity standards, and create opportunities for staff to grow into leaders.

Drive results with data

- Build dashboards and scorecards, and report regularly to the VP of Sales.
- Take part in executive leadership meetings and help shape growth initiatives.

How Success Is Measured

You’ll own key performance indicators including referral response time, referral-to-admission conversion and turnaround, management of pending referrals, analysis of lost referrals, follow-up compliance, documentation accuracy, communication with referral sources, and staff productivity.
