About this role

At Wayfinder Family Services , we understand the unique challenges facing some of our states most vulnerable children, youth and adults. Those with disabilities, those without a home of their own, those who have been abused and many, many more. We answer the call for them. We believe in the amazing potential in, and for, each and every one of them. And, together, we find a way to turn that potential into reality.

Position and Role Summary:

Wayfinders ATLAS program connects Sacramento County children, youth, and families to community outreach, early intervention, outpatient mental health, and Therapeutic Behavioral Services that improve access to care, strengthen families, and support long-term stability.

The primary focus of the Program Manager is to provide operational leadership for the Outreach and Engagement and Early Intervention components of ATLAS, Wayfinder's integrated children's specialty mental health services program under contract with Sacramento County Behavioral Health Services, and to hold accountability for community outreach and linkage performance, Community Health Worker and peer workforce development, Early Intervention program delivery, and accurate funding stream classification across MH-MAA, BHSA Early Intervention, and CalAIM BH-CONNECT Community Health Worker services, with peer support services aligned to Medi-Cal requirements.

The salary range reasonably expected for this position is $84,000 - $88,000.

Primary Responsibilities:

- Directs the day-to-day operations of the Outreach and Engagement and Early Intervention components, which function as the community-facing entry point to ATLAS services.

- Supervises the Community Health Worker team, the Peer Support Specialists, and other assigned outreach staff, provides weekly individual and group supervision on outreach and peer practice, documentation, professional boundaries, and field safety, and adjusts field assignments in response to referral volume and demand across the priority ZIP codes.

- Coordinates warm handoff of community referrals to the ATLAS intake team consistent with the No Wrong Door policy under BHIN 22-011, ensures staff who administer the DHCS Youth Screening Tool are trained and complete it consistent with DHCS requirements, and ensures screening results are routed to the licensed intake team for access determination and to the Managed Care Plan for non-specialty mental health services where indicated.

- Directs the peer support function, ensuring Peer Support Specialists hold and maintain current Medi-Cal Peer Support Specialist certification and continuing education, that Peer Support Services claimed to Medi-Cal are delivered under the direction of the designated Clinical Program Manager as the Behavioral Health Professional and documented to Medi-Cal, County, and Mental Health Plan standards, that peer staff are deployed to engagement, re-engagement, and family navigation where lived experience strengthens linkage, and that peer work is coordinated with each youth's primary clinician and Child and Family Team.

- Manages the Community Health Worker weekly field schedule across the fourteen priority ZIP codes, concentrating coverage where unmet need is highest based on County-shared ZIP-level unmet-need indicators and partner-site referral volume, and convenes the monthly outreach meeting tracking contact-to-linkage conversion.

- Oversees delivery of Early Intervention services for youth under 21 showing early need but not yet meeting SMHS medical necessity, including the Blues Program and Wayfinder's Be WELL curriculum, ensures facilitators are trained to each model, ensures Early Intervention remains distinct from Outreach and Engagement by activity type, tracks outcomes through client and parent satisfaction measures, and ensures participants who present with clinical concerns, safety risk, or possible SMHS need are referred to intake with clinical consultation from the designated Clinical Program Manager.

- Manages Outreach and Engagement presence and scheduling at approved co-location partner sites, including evening or weekend hours as committed, coordinates staff orientation with partner leadership, and maintains referral pathways with each partner.

- Identifies housing, transportation, and basic-needs barriers among families engaged through Outreach and Engagement and Early Intervention, links families to community resources, and refers youth who enroll in SMHS to the assigned Clinical Program Manager for Flexible Stabilization Supports, which are limited to youth actively receiving SMHS under an authorized treatment plan.

- Ensures documentation supports the funding stream claimed and meets MH-MAA, BHSA, BH-CONNECT, Medi-Cal peer support, County, and Mental Health Plan standards, and monitors outreach volume, staff productivity, and revenue performance, taking corrective action where performance trends below target.

- Supports the Program Director in County monitoring, audits, chart reviews, and corrective action plans, and implements resulting corrective actions within the Outreach and Engagement and Early Intervention components.

- Performs other duties as may become necessary or as directed.
