About this role
We are searching for a Care Coordinator Assistant. Someone who will provide non-clinical administrative support to the Care Coordination process including but not restricted to timely discharge planning and utilization management.

- Some college coursework preferred
- 2 years' experience in hospital care coordination/case management, home health, quality improvement or a related field required
