Bring your skills to patients who need compassionate care in their own homes. Elara Caring is hiring a PRN Licensed Practical Nurse to serve patients throughout Summit and Portage counties .

Work alongside an RN Case Manager and a collaborative care team to help patients manage their health, build confidence, and remain safely at home.

What You’ll Do

- Provide skilled nursing care according to physician orders and each patient’s individualized care plan.
- Monitor patient progress and report changes in condition or safety concerns.
- Teach patients and caregivers self-care techniques that support health and independence.
- Partner with the RN Case Manager as patient needs change.
- Maintain accurate medication profiles and complete timely clinical documentation.
- Communicate with physicians, patients, families, and the care team.
- Help reduce avoidable hospitalizations through proactive care and patient education.

What You’ll Need

- Current Ohio Licensed Practical Nurse license .
- Graduation from an accredited certificate, diploma, or associate nursing program.
- At least 1 year of experience in a clinical care setting .
- Home health or hospice experience preferred .
- Ability to frequently lift, push, pull, and support up to 50 pounds, including assisting with patient positioning, transfers, and equipment.
- Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
