Care Coordinator

What You’ll Do
• Coordinate new client intakes, caregiver scheduling, and ongoing care needs.
• Serve on the on‑call rotation: triage caregiver call‑outs, fill urgent shifts, and resolve client or family concerns after hours.
• Marketing Opportunities: Identify referral opportunities and assist with marketing initiatives.
• Assist with our hiring process including interviewing, onboarding, and checking references.
• Support our caregivers and ensure quality follow-up
• Monitor client satisfaction and follow up on any issues or concerns compliance with care plans.
• Conduct on-site visits and introductions

What You’ll Bring
• 1 year home care or home health care office experience preferred.
• Strong phone and communication skills, friendly, reliable, and detail-oriented
• Reliable transportation, valid driver’s license, and comfort using mobile apps / EMR systems
• Strong attention to detail and multitasking ability.
• Respond to incoming calls, inquiries, and service requests promptly
• Ability to lift / transfer clients and adhere to all health and safety guidelines
• Willingness to participate in an on-call rotation
