Job Description
BrightStar Care is seeking compassionate and dedicated Registered Nurses (RNs) to join our skilled home health nursing team!

What You'll Do
- Perform skilled nursing visits in patients' homes, providing individualized care according to physician orders and the established Plan of Care.
- Conduct comprehensive nursing assessments, monitor patients' conditions, and identify changes requiring physician notification or additional interventions.
- Provide skilled nursing treatments, including wound care, dressing changes, medication administration, injections, and other physician-ordered procedures.
- Manage and monitor patients with chronic illnesses, post-surgical needs, and other conditions requiring skilled nursing care.
- Perform medication reconciliation, assess medication effectiveness, and educate patients and caregivers on proper medication management.
- Provide patient and caregiver education on disease management, infection prevention, safety precautions, and overall health maintenance.
- Perform wound assessments and provide wound care treatments, including Wound VAC therapy when ordered and appropriately trained.
- Assist with IV therapy, PICC line care, catheter care, and other specialized nursing procedures based on patient needs, competency, and training.
- Communicate with physicians, case managers, therapists, and other healthcare professionals to ensure coordinated patient care.
- Complete accurate and timely clinical documentation, including skilled nursing visit notes, assessments, and updates to the Plan of Care.
- Report significant changes in patient conditions to the appropriate physician and agency clinical leadership.
- Maintain compliance with agency policies, infection control practices, and applicable home health regulations.

What We'll Love About You
- Current TX RN license
- Desire to provide home health service to pediatrics and adult patients
