About this role
JOIN THE META CARE CLERGY CARE TEAM
Patient Navigator / Care Coordinator
Clergy Care | Diocese of Rockford, Illinois
FULL-TIME • $55,000–$60,000 ANNUALLY • ROCKFORD, ILLINOIS
Know how to keep a busy medical practice moving? Bring those skills directly to the people who need them.
Meta Care is seeking an experienced medical-office or patient-care professional to support priests throughout the Diocese of Rockford. We are especially interested in someone who has worked in—or helped manage—a busy physician practice, specialty office, outpatient clinic or similar healthcare setting.
If you have been the person who handles referrals, follows up on tests, coordinates appointments, navigates prior authorizations, communicates with physicians and keeps patients from getting lost in the process, this may be an excellent next step.
## Your experience makes the difference
You understand how provider offices work, where delays happen and whom to call to get questions answered. In Clergy Care, you will use that practical experience to advocate for priests, coordinate their care and build trusted relationships through personal contact—not simply work behind a front desk.
## What you will do
• Serve as the primary local point of contact for priests who need help navigating healthcare services.
• Coordinate appointments, specialty referrals, testing, follow-up visits and communications with provider practices.
• Help members understand insurance requirements, prior authorization processes, medication access and other logistical barriers; work with the appropriate professionals when clinical or coverage decisions are required.
• Support transitions after hospitalization, rehabilitation or emergency department visits by coordinating next steps and needed services.
• Make regular outreach calls and in-person visits at parishes, homes, hospitals and other care settings.
• Maintain trusted relationships with local practices and create a practical network of healthcare resources.
• Document member needs and completed follow-up clearly; coordinate with Meta Care’s nurse care managers and specialty team as appropriate.
• Respect each member’s privacy, dignity and independence.
## What you will bring
• At least two years of hands-on experience in a busy physician practice, specialist office, outpatient clinic, patient navigation or care coordination setting.
• Experience as a Medical Assistant (MA), Certified Nursing Assistant (CNA), practice coordinator, referral coordinator, patient navigator, medical office lead or similar role is encouraged.
• Familiarity with appointment scheduling, provider referrals, insurance verification, prior authorizations and coordinating next steps for patients.
• Excellent communication, organization, problem-solving and follow-through; comfortable working independently.
• Comfort with Microsoft Office, electronic health records or medical office systems, and learning a care-management platform.
• No RN or LPN license is required. This position does not involve diagnosing, prescribing or independently providing clinical treatment.
• Ability to travel throughout the Diocese of Rockford for in-person visits.
• Commitment to serving in a manner consistent with Catholic Social Teaching and the Ethical and Religious Directives for Catholic Health Care Services.
Successful completion of required background checks and screenings is required.
## Compensation and benefits
• Annual salary: $55,000–$60,000
• Medical, dental and vision insurance; Meta Care pays 90% of individual medical insurance premiums
• Retirement plan; seven paid holidays; vacation and sick leave
