About this role

Licensed Practical Nurse (LPN) - Home Health

Serving patients across Merrillville and surrounding areas

Full - Time

Be Valued for the Nurse You Are

You became a nurse to care for people, not to race the clock. In home health, every visit is focused on one patient, and the relationships you build with patients and families become a real part of their healing.

That's our commitment to every Home Health LPN on our team: when you come to work with us, we go to work with you. You won't carry this work alone. You'll have a director who knows you, leaders who listen, and a team built to support you, so nursing can feel like nursing again. That means a real person when you call, help when you need it, and recognition for the difference you make every single day.

Why Interim HealthCare

We believe that when nurses are supported, they are free to do what they do best: care for patients. That is why our local leaders stay connected to our nurses, listen to their feedback, and make support part of how we work every day.

What That Means for You

- Market competitive visit rates

- Leaders who know you and recognize the work you do

- Schedule flexibility so your career fits your life

- Meaningful patient relationships with time to listen, teach, and follow through

- Training and growth through online training, in-services, continuing education opportunities, and tuition discount programs

- 401(k) up to 4% with immediate vestment to you!

What You'll Do

- Deliver physician-ordered nursing care in line with each patient's plan of care

- Contribute to ongoing interdisciplinary assessment and evaluation of the plan of care

- Monitor patients closely and report changes in condition to the RN supervisor

- Support the RN or physician with specialized procedures

- Teach patients and families self-care techniques and how to manage healthcare needs

- Document visits clearly and on time, including patient progress

- Collaborate with physicians and the care team to support continuity of care and discharge planning

What You Bring

- Graduate of an accredited practical nursing program with an active LPN license

- Home health experience preferred

- Current CPR certification for adults, children, and infants

- Valid driver's license, auto insurance, and reliable transportation
