Job Summary:
The Director of Rural Health Partnerships is responsible for the hospital and community partner network supporting Sharecare’s participation in Georgia’s Rural Enhancement and Transformation of Health (GREAT Health) Program. This role brings together the rural hospitals that anchor each market and the trusted local partners who carry AskMD into their communities, including faith leaders, pharmacists, clinics, health departments, extension agents, and civic organizations, so that hospital and community engagement work as one system.
Reporting to the Vice President, Rural Health Transformation, the Director is the program’s primary point of contact for hospital executives and community partner leaders, represents Sharecare with rural hospital and health associations, and leads the program’s Regional Field Managers. The role works closely with the Director of Rural Health Activation.
Essential Job Functions:
• Partner directly with each rural hospital spanning critical access, community, sole community, and rural referral center designations, and document each hospital’s participation and partnership.
• Build executive and clinical relationships at each hospital.
• Build and manage community partner networks around each hospital market, across faith and health ministries, federally qualified health centers, rural health clinics, health departments, pharmacies, Cooperative Extension, civic organizations, and community nonprofits.
• Manage each hospital market’s combined activation plan and budget, including implementation planning, staff and provider education, activation stations, patient materials, launch events, and local community partner activation funds.
• Align AskMD engagement with each hospital’s service lines, referral pathways, and priorities, including readiness for the CMS AHEAD Model and other value-based programs, and connect local partners to those pathways.
• Negotiate and manage hospital and community partnership agreements and memoranda of understanding, including data-sharing, consent, and co-branding terms.
• Design and deliver hospital staff and provider education and the trusted-messenger training program, so that front-line hospital teams and local messengers can introduce and refer residents to AskMD.
• Lead, hire, and manage two Regional Field Managers who oversee the program’s presence in each region of Georgia.
• Build Medicaid and family-serving channels, such as WIC, Head Start, school-based clinics, and food assistance providers, in coordination with DCH guidance.
• Define hospital catchment areas with DCH and maintain the reconciliation process that links county-level activation to hospital- and partner-attributed reporting.
• Coordinate launch sequencing and partner-hosted events with the Director of Rural Health Activation.
• Track hospital- and partner-attributed registrations and engagement, and focus resources on the partners and markets that perform.
• Capture hospital and community feedback, trust barriers, risks, and service gaps; escalate to the Vice President as needed; and manage a partnership opportunity fund for emerging local opportunities.
• Support DCH site visits, data assessments, and program milestone deliverables that involve hospital and community partners.
• Document the hospital onboarding model, partnership templates, agreement templates, and training curricula used to expand to additional Georgia hospitals, counties, and other states.
Specific Skills/ Attributes:
• Deep understanding of rural hospital operations, finance, and leadership priorities, and of community health strategies and social drivers of health.
• Exceptional executive relationship-building, negotiation, and facilitation skills.
• Proven ability to build trust and lasting partnerships with hospital, community, faith, and public health organizations.
• Strong program management and budget management skills.
• Knowledge of value-based care, population health, and Medicare and Medicaid reimbursement.
• Ability to lead and coach community-based staff and to lead through influence across hospital partners and internal teams.
• Cultural competence and comfort working across rural, racially diverse, and Spanish-speaking communities.
• Clear, credible communication with CEOs, CFOs, CNOs, physicians, and community leaders.
• Data-informed, execution-focused approach to measuring partnership results.
Qualifications:
• Bachelor’s degree required; MHA, MPH, MBA, MSW, or related advanced degree preferred.
• 10+ years of experience in hospital operations, health system partnerships, provider relations, community partnerships, or public health, including senior-level relationships with hospital executives and 3+ years leading staff.
• Direct experience working with rural, critical access, or community hospitals required.
• Demonstrated success building multi-site, multi-sector partnership networks with budget and contractual accountability.
• Experience designing or running trusted-messenger, community health worker, or lay health educator programs preferred.
• Familiarity with Georgia’s rural health landscape, DCH, and rural hospital associations strongly preferred.
• Experience with value-based care models and digital health or care navigation solutions preferred.
• Spanish-language proficiency preferred.
• Ability to travel frequently throughout Georgia, including evenings and weekends.
