Care Coordinator, Rochester, NY
Priority Cares Home Services, Rochester office (downtown Rochester)
You would make sure every Rochester client gets the right care at the right time. You would take new referrals, coordinate with families, managed care plans and doctors, schedule aides, follow up after every start of care, and solve problems the same day they come up.

What you need
• At least 1 year in care coordination, case management, intake or home care scheduling
• A nursing, social work or human services background is a strong plus
• Clear writer, careful with records, kind and firm on the phone
• Knowledge of Medicaid managed long-term care (MLTC) is a plus
