Position Overview
365 Health Services is seeking compassionate, dependable, and dedicated Certified Nursing Assistants (CNAs) to provide quality in-home care to our clients throughout Eastern Pennsylvania.
As a CNA, you will play an important role in helping clients maintain their independence, dignity, safety, and quality of life while receiving care in the comfort of their own homes.
The ideal candidate is caring, patient, professional, and passionate about making a difference in the lives of others.
Essential Duties & Responsibilities
- Provide or assist clients with personal care activities as outlined in their individualized plan of care, including bathing, grooming, dressing, hygiene, mobility, and toileting.
- Assist with activities of daily living (ADLs) while promoting client independence and dignity.
- Immediately report any changes in a client's condition or any task that cannot be safely performed to the Registered Nurse (RN) or Director of Clinical Services.
- Practice universal infection control precautions during all interactions with clients.
- Assist clients and families with home maintenance activities as outlined in the care plan, including light housekeeping and meal preparation.
- Utilize proper body mechanics and safe techniques for transfers, ambulation, range of motion, and positioning.
- Respect the privacy, confidentiality, autonomy, and rights of all clients and their families.
- Accurately document care provided and client/family responses, submitting all required documentation in a timely manner.
- Perform duties within the limits of your training, qualifications, and experience.
- Follow all applicable local, state, and federal regulations, Medicare requirements when applicable, and established agency policies and procedures.
- Consistently meet or exceed 365 Health Services' established service standards.
- Conduct all business activities in a professional and ethical manner.
- Maintain regular attendance and communicate scheduling concerns promptly.
- Adhere to the 365 Health Services Code of Conduct.
- Interact positively and professionally with all staff while supporting the agency's mission and vision.
- Complete required general orientation and ongoing training.
- Comply with all agency policies and procedures, including HIPAA and confidentiality guidelines.
Qualifications
- Current and valid Certified Nursing Assistant (CNA) certification.
- Previous healthcare, home care, or caregiving experience preferred.
- Compassionate, patient, and dependable with a genuine passion for helping others.
- Strong communication and interpersonal skills.
- Ability to work independently and follow individualized plans of care.
- Reliable transportation and ability to travel to client homes.
- Ability to meet applicable background screening and employment requirements.
