Access TLC is currently seeking a HCBA Registered Nurse (RN) to join our team in the HCBA department. The Home and Community Based Alternatives (HCBA) Waiver provides care management services to persons at risk for nursing home or institutional placement. The care management services are provided by a multidisciplinary care team comprised of a nurse and social worker. The care management team coordinates Waiver and State Plan services (e.g., medical, behavioral health, In-Home Supportive Services, etc.), and arranges for other available long-term services and supports available in the local community.
The HCBA Registered Nurse (RN) is responsible for assessing, care planning, assisting with locating, coordinating, and monitoring services for community-based participants on the waiver. This is a per diem position serving the Santa Ana area. Please note that orientation will be held in person at our Moorpark office, located at 5401 Tech Circle, Moorpark, CA 93021.
Visit Rates for Per Diem:
Initial assessment/reassessment visits: $150/visit
Level 3 Follow Up Visits: $50/visit
POT Phone: $75/visit
$50 per hour for orientation, training, meetings hours, and reimbursable travel time
Mileage is also reimbursed.

Requirements :
- Registered nurse with current licensure to practice professional nursing in the State of California.
- Minimum of one year nursing experience.
- Demonstrates excellent observation, verbal and written communication skills.
- Possess and maintains current CPR certification.
- Must have a California Driver’s License, satisfactory driving record and a vehicle that is insured in accordance with state/or organization requirements
