Key Responsibilities
- Help develop and carry out care plans for residents.
- Supervise and assist Certified Nursing Assistants (CNAs).
- Monitor residents' health and safety, including falls and skin care programs.
- Provide direct patient care and assist with daily medical needs.
- Complete and update resident evaluations and documentation.
- Support overall care goals and perform other duties as needed.
Qualifications
- Current Licensed Practical Nurse (LPN) license in good standing.
- Experience in skilled nursing or rehabilitation preferred.
