Southern Health Partners is launching a new correctional healthcare program in Cook County, Georgia , and were looking for a dependable Licensed Practical Nurse (LPN) to step into the Medical Team Administrator (MTA) role.

As the LPN MTA, youll be an important part of establishing a consistent, organized, and patient-focused healthcare program from the start. Youll provide hands-on nursing care while helping coordinate the day-to-day healthcare needs of the facility.

This is an opportunity for an experienced LPN who enjoys autonomy, organization, teamwork, and making a difference .

๐ŸŒŸ What Youll Do

- Provide direct nursing care to patients in a correctional healthcare setting

- Complete nursing assessments and monitor patient conditions

- Administer medications and treatments as ordered

- Respond to urgent and emergent healthcare needs

- Coordinate daily healthcare activities and help maintain continuity of care

- Communicate with medical providers, correctional staff, and SHP leadership

- Maintain accurate and timely clinical documentation

- Support compliance with company policies, clinical standards, and regulatory requirements

- Help establish organized and consistent healthcare processes at the new site

- Promote a professional, respectful, and patient-focused healthcare environment

Qualifications

๐ŸŽฏ What Youll Bring

- Current, unrestricted Georgia LPN license

- Strong clinical assessment and critical-thinking skills

- Excellent communication and organizational abilities

- Ability to work independently and confidently make sound clinical decisions

- Dependability and professionalism

- Ability to collaborate effectively with a multidisciplinary team

- Previous leadership or healthcare coordination experience is a plus

- Correctional healthcare experience is welcome, but not required
