About this role

STAFFING:

- Provide leadership & supervision of all direct care staff daily

- Ensure resident care is delivered in accordance with physician orders, care plans, and established standards of practice to maintain safe, high quality, resident-centered care

- Monitor direct care staff performance of health related activities, including the identification of any significant gaps in the staff person's ability to function competently

- Assist on the floor with routine needs, in the capacity as it relates to nursing or supervising issues. (Passing medications, documentation, issues with doctors or medical staff, coordination of appointments and therapies, assisting of families with their issues and needs)

- Oversee breaks and lunchtime of all direct care staff to ensure that proper coverage is always maintained

- Ensure direct care staff wear follow dress code, have appropriate footwear, & wear proper identification

- Ensure appropriate staffing levels to meet resident needs, manage scheduling (daily assignments and monthly schedule), and minimize overtime

- Knowledgeable of the schedule for all Direct Care staff and monitor hours

RESIDENTS:

- Plan, implement and evaluate the residents daily and on-going needs

- Oversee the daily needs & care of all residents, including hygiene and bathing issues, coordination with the dietary staff for resident meals, while also being aware of medical needs and monitoring medications

- Be proactive in building relationships with the residents so that they feel that they have an advocate and a support system within the facility

- Admissions: assist with setting up charts, assists residents and families with locating and settling into their room and orientating them to the facility

- Assists resident and families during discharge ensuring all belongings are packed, extra medications are returned to the pharmacy and resident charts is broken down for filing

- Monitor clinical outcomes and implement quality improvement initiatives

DOCUMENTATION:

- Review, monitor, and follow up on all daily documentation in the resident's file (functions, activities, illness, concerns, etc) completed by the RMA & aides

- Maintain documents of all resident records & filing of such documents

- Perform & ensure accurate and timely clinical documentation daily

- Ensure a system is established to document and monitor nursing assessments, medical visits, and order changes

- Review & complete Incident Reports in a timely manner

- Complete fall screenings upon admission, annually, and with any change in condition

- Complete UAI / ISP / TB screening / Fall Screening on all residents upon admission and at least annually thereafter, and as needed, to maintain compliance & meet resident needs

- Recommend and implement, in writing, changes to a resident's UAI & or ISP (service plan) based on needs whenever the plan does not appropriately address the current health care needs of the resident

MEDICATION:

- Monitor and audit medication carts, MAR logs and administration of resident medications

- Perform medication pass audits as outlined in the facility policy

- Knowledge of & an understanding of medication administration procedures and current medical issues facing the elderly population

- Ensure all medication orders are accurately approved and followed in EHR system for administration per provider orders

- Implement and follow all provider orders (including medications, labs, diets

- Oversee all narcotic/scheduled drug disposals

SUPERVISOR & GENERAL DUTIES:

- Carry out the daily routine as set forth by the Administrator in accordance with the needs of the facility

- Perform rounds of the facility each morning and either contact the appropriate staff to deal with the areas found or report it to appropriate facility department to be corrected

- Acts as a liaison & primary contact for all medical needs of residents including providers, hospitals, therapists, outpatient/outside vendors (hospice, home health, DME, etc)

- Act as the administrator when the Administrator is not present at the facility. This also includes dealing with family issues and resident care meetings

- Be available to oversee smooth operation & functionality of the facility in relation to resident/family concerns, problem areas, & visitors

- Keep Administrator and/or Corporate Designee informed on issues that arise with staff, residents, and families

- Collaborate with the Administrator and interdisciplinary team on operational planning and decision-making

- Handles sensitive information with the highest degree of integrity and confidentiality

- Foster a culture of accountability, teamwork, and professional growth

- Strong clinical judgment, problem-solving abilities, communication, and interpersonal skills

- Ability to manage multiple priorities in a fast-paced environment

- Knowledge of electronic health records (EHR) and healthcare documentation systems

- Assisting in the general cleanliness and the appearance of the facility (Resident rooms, common areas, chart rooms, clinics, dining areas)

- Ability to communicate and understand the physical and mental needs of the elderly

- Facilitate and run clinic appointments with providers (MD/NP, podiatry, Psych)

- Interviews prospective direct care staff, as needed

- Order medical supplies, as needed

- Assists in the implementation of all policies & procedures of the facility and regulations in conjunction with the DON/Administrator/Corporate Designee

- Ensure operations of the facility meets the regulations for Assisted Living set forth by the Department of Social Services for the state of Virginia

- Attendance is required during inclement weather

- Follows & implements infection control policies & procedures

- Follows, implements & enforces all policies & procedures of Runk and Pratt

- Maintains appropriate & respectful conduct within the workplace

- Provides support in any area that may arise that is above and beyond the normal expectations for this position

OTHER:

- Uniforms must be neat and clean, closed toe shoes are required

- Must have basic computer skills

- Must be able to perform duties in accordance with the guidelines mandated by Social Services as the licensing agent of this facility

- Be of acceptable physical and mental health to carry out assigned duties

- Possess mature judgment skills and is able to respond to emergency situations

- Provides support in any area that may arise that is above and beyond the normal expectations for this position.

- Strong attention to detail and time management skills

- Strong verbal & written communication skills

- Knowledge of basic principles of management and leadership

Qualifications

- LPN/RN in good standings as evidenced by the Virginia Board of Nursing and Virginia Department of Health Professions, without restrictions

- Management Experience

- A minimum of 6 months experience in either a long term or assisted living setting

- Complete annual in-service training as required

- State Police background check that is free from barrier offenses

- The ability to obtain CPR/FA training within 60 days of hire and maintain a current certification

- A current Tuberculosis screening on or within 7 days prior to the first day of employment and free from Tuberculosis

- The ability to meet physical requirements for position

- The ability to pass a drug test
