Job Summary:
The MAT Case Manager coordinates and supports patients receiving medication-assisted treatment for substance use disorders. They act as a liaison between patients, prescribers, nursing staff, and community resources to ensure continuity of care and promote long-term recovery.
Essential Functions:
- Conduct intake assessments and develop individualized treatment and recovery plans.
- Monitor patient progress, adherence to prescribed medications, and overall treatment goals.
- Provide counseling, education, and referrals to supportive services (housing, employment, peer support, etc.).
- Coordinate care with physicians, nurses, therapists, and outside providers.
- Maintain accurate, timely documentation in compliance with program and regulatory standards.
- Advocate for patients needs while promoting harm reduction and recovery-oriented approaches.
- Ensures compliance with Federal, State, and local regulations and RiverValleys mission policies, procedures and standards.
- Completes all mandatory compliance, HIPAA and state and federal regulatory training as required.
- Maintains an appropriate degree of confidentiality.
Qualifications
Education and Experience:
- Requires a background in social work, counseling, psychology, or a related behavioral health field.
- Bachelor's degree from an Accredited College in a Human Services field or a Bachelor's degree in a different discipline with two (2) years' experience working in a health care setting, behavioral health setting preferred.
- Valid driver's license, reliable operating transportation, and proof of current auto insurance
- Prefer experience working with children, families, and adults who experience complex emotional and behavioral difficulties which may present in crisis.
Knowledge and Skills:
- Knowledge of substance use disorders and evidence-based treatment approaches.
- Strong organizational and case management skills.
- Ability to build rapport and maintain professional boundaries with patients.
- Familiarity with community resources and support systems.
