Job Title: Inpatient Pharmacist Job Description This Inpatient Pharmacist role is designed for clinicians who thrive in a pharmacist-as-practitioner model and are comfortable operating with a high degree of autonomy. The position carries a strong emphasis on complex medication reconciliation, clinical order review, and direct therapeutic recommendations—including making medication changes on behalf of the hospitalist group in alignment with established protocols. Pharmacists in this role are integrated members of the care team and are expected to exercise advanced clinical judgment in a fast-paced acute care environment. Responsibilities
- Conduct complex medication reconciliation for high-acuity patients across multiple service lines.
- Perform clinical order evaluation and optimization, including recommending and implementing therapeutic changes under collaborative practice expectations.
- Manage pharmacotherapy for acute and chronic disease states.
- Oversee IV preparation and collaborate on sterile compounding with technical staff.
- Engage in interdisciplinary collaboration with hospitalists, nursing, case management, and ancillary teams.
- Exercise autonomous clinical decision-making within established protocols and scope.
- Manage documentation and workflow within Epic.
- Support medication distribution systems including Pyxis and Carousel.
Essential Skills
- Active Virginia Pharmacist License in good professional standing.
- PharmD or B.S. Pharm (PharmD preferred).
- Recent acute care experience (within the last 3 years).
- Epic experience required.
- Strong ability to work independently and manage complex clinical scenarios.
Additional Skills & Qualifications
- ASHP-accredited PGY-1 Residency.
- Experience in computerized hospital pharmacy systems.
- IV sterile compounding and advanced pharmacotherapy experience.
- Familiarity with Pyxis, Carousel, and Epic medication workflows.
- Excellent communication and interpersonal skills.
Work Environment This role operates within a fast-paced acute care environment that emphasizes teamwork and collaboration. Pharmacists are expected to make autonomous clinical decisions while working closely with hospitalists and other healthcare professionals. The use of technologies such as Epic, Pyxis, and Carousel is integral to the workflow, requiring proficiency and experience in these systems.

Pay and Benefits
The pay range for this position is $65.00 - $67.00/hr. Individual compensation offered for this position within this range will depend on many factors, including qualifications, skills, relevant experience, job knowledge, geographic location, internal equity, and other pertinent job-related factors.
