Clinical Supervisor (RN) – Home Health | Oahu

Always Best Care is growing, and were looking for an experienced Registered Nurse (RN) to join our leadership team as a Clinical Supervisor .
This is an opportunity for an experienced home health nurse who is ready to take the next step in their career. Youll help lead and support our clinical team, strengthen patient care, and work closely with our Director of Nursing to keep our growing agency operating at a high clinical standard.
This is not strictly a desk position . Youll stay connected to patient care through supervisory visits, clinical support, and occasional direct patient visits while also helping develop and mentor our field clinicians.

💼 What Youll Do
Lead & Support Our Clinical Team
- Supervise, mentor, and support field clinicians
- Serve as a clinical resource for RNs, LPNs, therapists, and home health aides
- Assist with orientation, training, competencies, and staff development
- Conduct supervisory visits and provide coaching when needed
Support Excellent Patient Care
- Review assessments, plans of care, and clinical documentation
- Help coordinate care for complex and high-risk patients
- Ensure care is delivered according to provider orders and the plan of care
- Perform patient visits, including SOCs, ROCs, supervisory visits, and other skilled nursing visits as needed
- Assist with field coverage when necessary to maintain continuity of care
Promote Quality & Compliance
- Review OASIS and clinical documentation for accuracy and timeliness
- Help ensure documentation supports skilled need and medical necessity
- Collaborate with the Director of Nursing and QA team on chart reviews and performance improvement
- Support compliance with Medicare, regulatory, and agency requirements
Help Our Agency Grow Stronger
- Participate in QAPI and performance improvement initiatives
- Identify clinical trends and opportunities for improvement
- Help strengthen workflows, communication, and continuity of care
- Support staffing and clinical operations
- Provide appropriate after-hours clinical availability and participate in on-call responsibilities as needed
✅ What Were Looking For
- Current Hawaii Registered Nurse (RN) license
- Graduate of an accredited nursing program
- Current CPR certification
- At least 1 year of home health experience required
- Leadership or supervisory experience preferred
- Strong clinical judgment and communication skills
- Comfortable coaching and supporting clinical staff
- Organized, dependable, and able to manage competing priorities
- Ambulatory care experience preferred
🚗 This Is a Hands-On Leadership Role
Our Clinical Supervisor plays an active role in the field. Candidates must have:
- Valid drivers license
- Reliable, insured vehicle
- Ability to travel throughout Oahu
- Ability and willingness to perform patient visits and provide clinical coverage when needed
