Health and Inclusion Project Worker, George's Hill Long-Term Accommodation, Dublin

Contract: Full Time, Permanent

The purpose of this role is to provide stability, specialist support and access to primary care and other health related services. The role will include, providing stability and specialist support for individuals based on a comprehensive assessment of health, mental health and addiction needs.

What you will do:

- Work within the objectives, service model, policies, procedures and standards of best practice of the Georges Hill & Stanhope Green LTH Service and the wider organisation.
- Provide a safe, welcoming and supportive environment for customers who are out of home or at risk of homelessness, including basic physical care and support while respecting customer choice, dignity and self-determination.
- Complete comprehensive assessments of customer needs, including health needs, and develop, implement and review individual support plans using a Care and Case Management approach where appropriate.
- Establish and maintain trusting and professional relationships with customers, particularly those experiencing homelessness or living in temporary accommodation, supporting them towards greater stability and independence.
- Ensure customers receive appropriate and ongoing support in relation to general health, mental health and substance use, including tailored one-to-one support in the areas of mental health and addiction.
- Provide advice, information and guidance to prevent or respond to homelessness and support customers to develop life skills and access education, training and employment opportunities where desired.
- Support customers to integrate within their local communities and access appropriate clinical, medical, counselling, health and other support services, including accompanying customers to appointments where required.
- Work collaboratively as part of a multidisciplinary service, liaising closely with other Focus Ireland teams, including Youth Housing Tenancy Support Workers, as well as statutory, voluntary and community agencies and relevant health professionals such as Public Health Nurses.
- Maintain an appropriate customer caseload as directed by the Line Manager and ensure accurate and up-to-date casework records, daily logs, written assessments and other required information are recorded on the Focus Ireland database and PASS.
- Develop and facilitate customer and peer-support groups where appropriate, based on identified customer needs and strengths, and ensure customers are consulted and involved in the operation and development of the service to the greatest practical extent.
- Work a flexible rota as agreed with the Georges Hill Service Manager and participate in team, section, divisional, agency and external meetings, handovers and training as required.
- Contribute to the planning, implementation, monitoring and evaluation of daily activities and service delivery, including reporting on implementation progress and contributing to the development and implementation of policies and procedures.
- Participate in regular supervision, training and professional development, and supervise students or volunteers when requested by the assigned supervisor or Georges Hill LTH Service Manager.
- Maintain awareness of Health, Safety and Welfare risks, promptly report concerns to the Line Manager or Health & Safety Representative, and undertake any other reasonable duties assigned in consultation with the Georges Hill LTH Service Manager.

What you will need:

- A Third Level qualification at degree level in the area of social care i.e. youth work, social studies, addiction, psychology, Nursing etc and/or be a CORU registered Social Care Worker.
- A recognised third level qualification in social care (minimum Level 8), social care, nursing, psychology, addiction studies or a related field.
- Two years’ experience of working with single homeless people, substance users, people experiencing poor general and/or mental health and/or other socially marginalised groups.
- Experience of working with customers at risk.
- Experience of team working.
- Report writing abilities.
- Experience of working and liaising with other agencies, including both voluntary & statutory.
