Perinatal Navigator - Job Overview

Schedule: Monday-Friday (8AM-4:30PM or 8:30AM-5PM)

As a Perinatal Navigator, you’ll have the opportunity to walk alongside pregnant individuals and families during some of life’s most meaningful moments. You’ll help patients find the care, education, and community support they need throughout pregnancy, postpartum recovery, and the early stages of their child’s life.

This entry-level role is a chance to build trusting relationships, reduce barriers to care, and make a lasting difference in maternal and infant health outcomes. Working closely with an integrated clinic and care team, you’ll use health education, goal setting, motivational interviewing, and other evidence-based techniques to encourage sustainable health behavior change.

Your compassion, organization, and commitment to patient-centered care will support Legacy’s Walk-In Pregnancy Testing services and the implementation of the Peripartum Navigation program. You’ll also help patients connect with Eligibility services, coordinate New OB, postpartum, and newborn appointments, and access essential resources that support healthy families.

Position funding may be partially or fully supported by grant(s), requiring alignment with applicable grant and contract deliverables.

- Maintain operational oversight of walk-in pregnancy testing activities, including testing patients and coordinating follow-up care.

Key Responsibilities

- Conduct point-of-care testing, including HIV testing, pregnancy testing, and related services.
- Conduct health screenings and risk assessments.
- Assist clients with scheduling patient appointments and follow-ups.
- Conduct outreach to identify and engage pregnant individuals in need of support.
- Provide culturally sensitive education on prenatal care, nutrition, and healthy pregnancy practices.
- Conduct brief screenings of patient needs, including behavioral, social, and medical needs, readiness for health behavior change, and progress over time.
- Assist patients to set goals related to their health.
- Implement comprehensive retention strategies for OB and pediatric patients, including postpartum follow-up, pediatric wellness continuity, and family engagement.
- Guide patients in navigating the system of care in order to achieve health goals.
- Connect clients with community resources such as WIC, housing, transportation, and behavioral health services.
- Document all patient services in the electronic health record in the Legacy EMRS.
- Serve as a member of an inter-disciplinary clinic and care team.
- Promote effective working relationships with internal and external stakeholders.
- Meet assigned performance metrics, including those related to grant/contract requirements, if applicable.
- Participate in clinic, team, departmental, and agency meetings, trainings, and workshops as directed, including travel to different Legacy clinics.
- Perform other duties as assigned.

Minimum Qualifications

- High School Diploma or equivalent is required.

Required Years of Experience

- Two (2) years of relevant experience is required.

Desirable Education and/or Experience

- Bachelor’s Degree in a related field with a minimum of 1 year of relevant experience is preferred.
- Previous experience working with OB and Pediatric patient populations.
- Previous experience providing point-of-care testing, working with sexual minorities and gender non-binary persons, and providing education about pregnancy and/or the postpartum period.

Required Knowledge, Skills, and/or Abilities

- Excellent communication skills and customer service skills. Effective communication skills, both oral and written.
- Basic understanding of statistics and graphical representation principles.
- Ability to manage multiple deadlines and prioritize tasks effectively.
- Demonstrated competency with computers and applications, including Microsoft Office Suite.
- This job requires access to confidential and sensitive information, requiring ongoing discretion and secure information management

Desirable Knowledge, Skills, and/or Abilities

- Bilingual English / Spanish fluency.
- Competency with Electronic Health Record System (EHRS).
