In this fully remote position, you'll use your clinical expertise to provide personalized Medication Therapy Management (MTM), support patients with chronic conditions, and collaborate with healthcare providers to improve outcomes. What You'll Do

- Conduct comprehensive medication reviews and MTM consultations by phone and telehealth
- Educate patients on medication use, adherence, side effects, and disease management
- Develop and monitor individualized medication care plans
- Identify and resolve medication-related concerns and barriers to treatment
- Collaborate with physicians, pharmacies, caregivers, case managers, and community health partners
- Assess patient needs and help connect them to appropriate healthcare and community resources
- Support patients managing chronic conditions and complex medication regimens
- Maintain accurate clinical documentation, including SOAP notes
- Serve as a trusted healthcare resource for patients who may have limited access to care

Qualifications

Required:

- Active Missouri Pharmacist License
- Missouri residency
- Strong communication and patient counseling skills
- Ability to establish trust and engage patients remotely
- Strong organization, time management, and documentation skills
- Ability to work independently in a remote environment

Preferred:

- MTM experience
- Medicaid, managed care, payer, or insurance experience
- Ambulatory care or population health experience
- Experience with chronic disease management
- SOAP note or clinical documentation experience
- Experience supporting medically complex patient populations

This is a Contract position based out of Saint Louis, MO.
Pay and Benefits
The pay range for this position is $50.00 - $55.00/hr. Individual compensation offered for this position within this range will depend on many factors, including qualifications, skills, relevant experience, job knowledge, geographic location, internal equity, and other pertinent job-related factors. Eligibility requirements apply to some benefits and may depend on your job
