Responsibilities
- Provide direct nursing care, medications, and treatments in accordance with physician orders and care plans.
- Monitor resident condition and promptly communicate changes to the healthcare team.
- Accurately document resident care and interventions.
- Collaborate with physicians, nurses, therapists, and interdisciplinary care teams.
- Follow infection prevention, safety, and regulatory standards.
- Deliver compassionate, resident-centered care that promotes health, dignity, and quality of life.
Qualifications
- Active, current, unrestricted Licensed Practical Nurse (LPN) license in the state of practice.
- Graduate of an accredited Practical Nursing program.
- New graduates are encouraged to apply.
- Experience in skilled nursing, long-term care, rehabilitation, or post-acute care is a plus but not required.
- Strong communication, critical thinking, and teamwork skills.
- Ability to provide compassionate, patient-centered care in a fast-paced healthcare environment.
