General Summary:
The Intake Team Supervisor provides clinical and operational leadership for the Intake Team. This role ensures timely, accurate, and patient centered intake processes across all referral sources, supporting smooth transitions into hospice and inpatient services. The Intake Team Supervisor partners closely with Admissions, Heathers House, and clinical operations to optimize referral management, throughput, and quality outcomes.
Essential Functions:
Operational Leadership
- Develop and maintain the monthly staff intake schedule, including PTO coordination and coverage planning.
- Approve end-of-the-month timesheets for staff in accordance with organizational policy.
- Serve as the primary operational resource for the Intake Team, ensuring daily workflow continuity.
Referral Management and Performance Oversight
- Monitor open referrals to ensure appropriate follow up, timely contact, and accurate disposition.
- Track and trend referral closures, including detailed documentation of closure reasons.
- Identify patterns related to delays, barriers, or missed opportunities and escalate concerns to leadership as appropriate.
Education and Staff Development
- Provide ongoing education and coaching to intake staff related to the intake process, referral triage, and communication standards.
- Assist with onboarding, training, and competency development of new intake staff.
Quality and Collaboration
- Collaborate with Admissions Patient Care Manager and Director of Clinical Operations to ensure alignment between intake and admissions workflows.
- Support continuous improvement initiatives related to access, response times, and referral conversion.
- Promote a patient and referral source centered approach that aligns with organizational values and regulatory expectations.
Education & Licensure:
- ADN or BSN, BSN Preferred
- Current Registered Nurse (RN) license in MN/ND
- Certification in Hospice and Palliative Nursing (CHPN) preferred or the ability to obtain within 1 year of starting role.
Experience:
- Minimum of 3 years of experience in a healthcare or hospice setting required, with progressive responsibility growth.
- Experience with patient admissions, documentation, and referral coordination preferred.
- Knowledge of hospice eligibility criteria, regulations, and compliance standards
