ABOUT THE ROLE:
SUMMARY:
Responsible for providing case management services to individuals who have been committed to a State Mental Health Treatment Facility (SMHTF) under the provisions of Chapter 394, F.S. Will track individuals during their state mental health treatment stay and post discharge to ensure treatment engagement, stable housing, resumption or acquisition of benefits, and meaningful involvement in the community. This position will be responsible for facilitating communication and collaboration between SMHTFs and behavioral health systems in order to coordinate appropriate services.

Essential Duties and Responsibilities :
The Duties and Responsibilities reflect the general details considered necessary to describe the principal functions of the job and shall not be construed as a detailed description of all related work assignments that may be inherent to the job.
- Work with the SMHTF to develop the psychosocial assessment and the recovery plan.
- Participate in monthly reviews of the recovery plan.
- Provide the SMHTF recovery teams with all available community information required to assist with the individual's treatment.
- Participate in the discharge planning meeting calls and assist in the development of a discharge plan which addresses the needs of the client in the community. Actively carry out linkage and brokerage activities in the community prior to the individuals discharge in order to implement the service plan.
- Secure community placement and services in cooperation with SMHTF social worker and discharge planner.
- Maintain at least monthly contact with the SMHTF staff concerning the status of the client.
- Maintain contact with the individuals family consistent with Chapter 394, F.S.
- Have a face-to-face contact with the individual in the community within 2 working days of discharge from the SMHTF.
- Coordinate Social Security office visit within five business days of discharge from the SMHTF for individuals who are discharged with benefits in pending status to ensure their benefits are activated.
- Maintain progress notes in the client record reflecting all meetings and communications.
- Participate in Managing Entity (ME) initiated conference calls and meetings, including monthly Seeking Placement List review calls.
- Maintain working relationships with local behavioral health providers and housing facilities.
- Performs other duties required by Supervisor.
Qualifications
CORE COMPETENCIES:
- Maturity and sensitivity to the special needs of those who have a co-occurring mental health and/or substance use disorder.
- Ability to communicate clients needs and information to behavioral healthcare staff.
- Ability to utilize and navigate an electronic health record.
- Ability to communicate clearly about the needs of the program to program director.
- Perform other duties as required by supervisor.
QUALIFICATION S, EDUCATION, AND/OR EXPERIENCE:
To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.

- Bachelor's Degree required in Social Work, Sociology, Psychology, or related Human Services field. Masters degree preferred.
- Minimum of two years experience working in a behavioral health environment required.
- Experience working and/or knowledge in a not-for-profit or government behavioral health business (preferred).
- As a 24-hour facility you are required to be available 24 hours a day/7 days a week.
- Maturity and sensitivity to the special needs of the mentally ill population
- Ability to assess needs of consumers.
- Good organizational, interpersonal and communication skills.
- Ability to work independently with minimal direction supervision.
- COMPUTER SKILLS: Must have Intermediate to advanced experience in MS Office Suite (Outlook, Word, Excel, PowerPoint), the use of personal computers, and standard office productivity software.
LANGUAGE SKILLS : Ability to read and interpret documents such as safety rules, operation and maintenance instructions, and procedure manuals
REQUIREMENTS:
- A valid Class E Drivers License for positions that require driving personal or Agency vehicle on Agency business.
- Must maintain a minimum Bodily Injury & Property Liability insurance limit of at least $50,000 for positions that require driving personal own vehicle (POV) on Agency business. (Proof Required)
- PPBH is a 24/7 Agency. All employees must be available as needed when required.
