Join Our Care Coordination Leadership Team!
Our BlueCare team is seeking a CHOICES Care Coordinator Supervisor for the Middle Tennessee Region with expertise in Medicaid, Long-Term Services and Supports (LTSS), case management, and leadership and a genuine passion for serving elderly and physically disabled individuals.
In this role, you will lead, mentor, and support a team of Care Coordinators responsible for managing the complex needs of CHOICES members across Middle Tennessee. You will oversee daily operations, provide clinical and operational guidance, monitor performance and quality standards, and ensure members receive the support and services needed to remain safely in their homes or nursing facilities.
As a Supervisor, you will play a critical role in driving member-centered outcomes, addressing barriers to care, ensuring contractual and regulatory compliance, and fostering a culture of accountability, collaboration, and excellence. You will serve as a key resource for staff, providing coaching, resolving escalated issues, partnering with providers and community stakeholders, and supporting organizational goals through effective leadership.
This position is ideal for a self-motivated and adaptable leader who thrives in a fast-paced, mission-driven environment. The Supervisor must be comfortable balancing multiple priorities, responding to changing business needs, and supporting both operational and member-facing activities.
In addition to supervisory responsibilities, there may be occasions when business needs require the Supervisor to temporarily carry a member caseload , particularly during staffing transitions or other operational circumstances. The ability to effectively manage direct member responsibilities while maintaining leadership oversight is essential to this role.
Key Highlights:
- Lead and support a geographically dispersed team of Care Coordinators serving CHOICES members throughout Middle Tennessee
- Provide hands-on guidance for complex, high-profile, and escalated member situations
- Collaborate with providers, nursing facilities, community partners, and internal departments to address member needs and service gaps
- Monitor performance, quality metrics, documentation standards, and regulatory compliance
- Coach and develop staff through ongoing training, case consultation, and performance management
- Assist with workforce planning, onboarding, workload balancing, and operational improvement initiatives
- Maintain accurate and timely completion of multiple reports, audits, and deliverables with strict deadlines
- Provide direct support during member crises, challenging family interactions, and critical care coordination situations
- Regular travel throughout the Middle Tennessee region as business needs dictate
- Area Covered: Bedford, Cannon, Coffee, Dickson, Giles, Hickman, Humphreys, Lawrence, Lewis, Lincoln, Marshall, Maury, Perry, Van Buren, Warren, and Williamson Counties

Job Responsibilities
- Monitor performance of staff including service performance and adherence to established utilization benchmarks. Provide and/or coordinate staff training for maximum performance and provide developmental opportunities.
- Supervise, direct, and evaluate a diverse group of health care professionals to assure effectiveness of consumer advocate, ancillary, discharge, institutional, care coordinator assistant and home and community-based care coordinator activities.
- May be required periodically to carry case load of members, when dictated by census fluctuations or staffing coverage.
- Responsible for communicating and mentoring members of the care coordination team to ensure long-term and home and community based care guidelines, policies, and procedures are followed.
- Valid driver's license.
- 90% day travel required.
- Employee may be required to participate in Runzhimer Program (auto reimbursement plan).
- Various immunizations &/or associated medical tests may be required for this position.
Job Qualifications
Experience
- 3 years - Supervisory experience required
- 2 years - Experience in the health insurance/managed care field required
Skills\Certifications
- Ability to conduct root cause analysis, plan, organize and coordinate multiple projects
- Proficient in Microsoft Office (Outlook, Word, Excel and Powerpoint)
- Excellent oral and written communication skills
- Strong interpersonal and organizational skills
License
- Registered nurse with an active license in the state of Tennessee or hold a license in the state of their residence if the state is participating in the Nurse Licensure Compact Act; or Masters in Social Work with an active unrestricted license.
