Description
Under the direction and guidance of the Chief Executive Officer, The Supervisor of Community Case Management is responsible for overseeing the administration, compliance, and operations of participating in Continuum of Care (COC) agencies under Housing and Urban Development’s (HUD) guidelines and advancing Valley Healthcare’s mission to end homelessness in the Chattahoochee Valley. This position is responsible for the development, implementation and evaluation of systems and processes related to partnerships in the community and case management. The Supervisor of Community Case Management is also responsible for ensuring that the department provides high-quality technical assistance along with effective resources within the community that are responsive to the needs of the clients, families, and other members in the community.
Supervisory Responsibilities:
- Recruiting, training, and managing community case management partners
- Setting goals for performance and deadlines in ways that comply with Continuum of Care’s
- (COC) plans and vision.
- Organizing workflow and ensuring that employees understand their duties or delegated
- tasks.
- Monitoring employee productivity and providing constructive feedback and coaching.
- Receiving complaints and resolving problems.
- Relaying information from upper management to employees and vice versa Ensuring the sub-granted agencies participating in the Continuum of Care (COC) adhere to
- the Housing of Urban Development (HUD) and UWCV compliance rules and regulations.
Duties/Responsibilities:
- Direct the development and implementation of departmental policies, procedures, and
- programs.
- Oversee the daily operations of case management with agencies actively participating with the
- Continuum of Care (COC) to ensure quality Case management and efficient workflow.
- Establish protocols and benchmarks for case management success
- Use benchmarking to demonstrate community impact
- Monitor staff performance and provide feedback and coaching to promote professional growth.
- Foster positive relationships with members of the agencies in Muscogee and Russell County.
- Advocate on behalf of client/ families to ensure their needs are met.
- Serve as a resource to case managers regarding best practices and ethical decision-making.
- Conduct audits to ensure documentation meets quality standards.
- Attend meetings and conferences as needed to represent the department.
Requirements
- Excellent communication skills, particularly listening, mediation, and writing skills.
- Possess strong organizational skills with ability to meet a demanding workload.
- Detail oriented to complete requirements of client files and grant reporting compliance.
- Ability to work collaboratively with other personnel and/or service providers or professionals.
- Creative thinker/adaptive personality.
- Able to read and interpret contracts.
- Valid driver’s license
- Experience with computers and knowledge of Microsoft Office, Outlook and Teams.
- Understanding of the commitment to the mission, vision, and goals of VHcS.
Education and Experience:
Bachelor of Social Work (BSW) degree preferred or a minimum bachelor’s degree in human service field and two years’ experience in the following:
- Case management experience with low-income adult clients and families
- Experience/Understanding of issues affecting homeless individuals and families.
- Knowledge and practice using community resources and referrals.
Evaluation: The evaluation of work performance, including: specific duties, responsibilities, demeanor, and work ethic, will be ongoing and implemented by the Supervisor per center policies.

Security (HIPAA): Every employee is required to protect their computer log-on and password. You must secure your work area prior to leaving it unattended for an extended period of time. You are required to log-off of your computer and secure your work area prior to leaving at the end of the day. If you have a responsibility for closing or locking files, securing equipment, and you do not comply with these requirements, you will be subject to reprimand and/or termination based upon the infraction. We are required by law to protect patient privacy, identifiable patient information, and all properties containing such information.
Physical Requirements: Prolonged periods of sitting at a desk and working on a computer. Must be able to lift up to 15 pounds at times.
