About this role
Job Purpose:
- The OPTUM program provides individuals with integrated behavioral health home (IBHH) services. These services make it easier for people with behavioral health challenges to get the support and tools they need. The Care Manager I functions as an advocate and support person to those individuals assigned within the program.
Key Responsibilities:
- Links consumers to appropriate services and monitors the progress or lack of progress by the individual.
- Works closely with the individual, the individuals family, LifeStream staff, and other community organizations to meet those needs and to allow the individual to live as independently as possible within the community.
- Convenes/attends staffing as necessary for assessing and coordinating services for clients.
- Completes required agency, funding, legal, statistical and fiscal documentation in an accurate and timely manner.
- Direct care services include assessment interventions and case management.
- Regular driving between appointments and a clean driving history and valid drivers license is an essential function of the position.

Qualifications
Required Education:
- Bachelors Degree with major course work in the human services field required
Required Experience:
- Minimum of two years experience in the social services field required
- If in personal recovery, two (2) years of successive recovery required
- Minimum of two (2) years of experience working with the chronically mentally ill in an acute psychiatric or case/care management function preferred
Required Skills and Abilities:
- Strong understanding of behavioral health principles and practices.
- Excellent communication skills, both verbal and written, with the ability to interact effectively with clients, families, and healthcare professionals.
- Ability to assess client needs and develop personalized care plans.
- Proficiency in using case management software and electronic health records.
- Strong organizational skills and attention to detail.
- Ability to work independently and manage time efficiently in a dynamic environment.
- Compassionate and empathetic approach to client care.
