Position Summary
The Clinical Risk Specialist leads the investigation of patient safety events and other significant organizational incidents. This position partners with operational leaders to conduct objective investigations, facilitate Root Cause Analyses (RCA), identify contributing factors, develop corrective action recommendations, and promote organizational learning that improves patient safety and reduces future risk.
The Clinical Risk Specialist serves as a subject matter expert in incident investigation methodologies and collaborates closely with Clinical Excellence, Accreditation & Quality Governance, Patient Safety Organization (PSO), Client Voice & Experience, and operational leadership.
Essential Duties and Responsibilities
- Conduct investigations of patient safety events and other reportable incidents according to established enterprise guidelines.
- Review assigned incidents and establish investigation plans.
- Determine scope and investigative approach based on event type and risk level.
- Prioritize investigations according to organizational and regulatory requirements.
- Interview team members and other involved individuals.
- Collect and analyze clinical documentation and supporting evidence.
- Facilitate Root Cause Analyses (RCA), Apparent Cause Analyses (ACA), and mortality reviews.
- Identify system issues and contributing factors.
- Develop recommendations and corrective action plans.
- Partner with operational leaders to implement corrective actions.
- Monitor corrective action implementation.
- Participate in enterprise safety committees.
- Collaborate with the PSO by providing incident information to support protected patient safety analysis.
- Provide consultation regarding incident reporting and investigation processes.
- Support organizational responses to regulatory inquiries, surveys, and requests for incident-related information.
- Maintain investigation documentation.
- Identify opportunities for organizational improvement.
- Perform other duties as assigned.

Minimum Qualifications
Education
- Bachelor's degree in Nursing, Behavioral Health, Healthcare Administration, Social Work, Psychology, or related healthcare field, or equivalent combination of education and relevant experience. Master’s degree is preferred.
Experience
- Three years healthcare experience.
- Experience in quality, patient safety, clinical operations, compliance, or risk management preferred.
- Experience participating in incident investigations preferred.
Preferred Certifications
- CPHQ
- CPPS
- Licensed clinical professional
Physical and Work Environment Requirements
Work is performed in an office, clinical, residential, and/or community-based environment depending on role and assignment. The position may involve routine use of office, clinical, or electronic systems and equipment. Duties may require prolonged periods of sitting, standing, walking, and working at a computer. Work hours may include evenings, weekends, holidays, or extended shifts based on program needs. This position is classified as sedentary work. Sedentary work involves exerting up to 10 pounds of force occasionally and/or a negligible amount of force frequently to lift, carry, push, pull, or otherwise move objects.
