$73K-$85K+ Annually depending on years of RN experience and education

The Integrated Care Registered Nurse - Mobile Health position provides and coordinates supportive services for the integrated patient population at Terros Health. This position supports the population seeking treatment for primary care, substance use disorder (SUD), general mental health, serious mentally ill and dual diagnosis. Engages with patients and works as part of the care team to close care gaps. Provides services through the Mobile Health Unit in community-based settings including shelters, schools, community centers, outreach locations, employer sites and other designated locations throughout Maricopa County. Supports population health initiatives, preventive screenings, vaccinations, chronic disease monitoring, outreach, care coordination, and transitions of care. Collaborates with providers, behavioral health staff, community health workers, and community partners to improve healthcare access and outcomes for underserved populations
Duties Include:
- Provides nursing services. Performs all duties required of an RN in an Integrated healthcare system.
- Maintains appropriate knowledge, skill, and certification for this position.
- Performs specialized duties as required by special clinical situations or treatment populations consistent with license and scope of practice, including providing patient engagement in treatment, treatment planning and patient education and support.
- Establishes therapeutic relationships with a variety of patients for the purpose of establishing and reaching appropriate health and treatment goals.
- Identifies potential barriers to quality care and opportunities for improvement and reports them to the Operational Supervisor and/or the Director of Nursing.
- Seeks appropriate clinical supervision to ensure the provision of effective treatment.
- Ensures documentation of patient encounters in accordance with policies, procedures, and clinical protocols same date of service.
- Coordinates medication access, patient follow-up, referral completion, and care transitions to support continuity of care and treatment adherence.
- Works collaboratively with site operational and clinical leaders to ensure an efficient workflow to address program needs.
- Supports and participates in huddles, site, medical staff and agency meetings.
- Provides community-based nursing care and health screenings within mobile health settings.
- Performs nursing triage, health assessments, point-of-care testing, specimen collection, vaccinations, and tuberculosis screening.
- Identifies and closes care gaps including immunizations, cancer screenings, chronic disease management, and preventive care services.
- Coordinates referrals and follow-up services for primary care, behavioral health, substance use disorder treatment, specialty care, and community resources.
- Supports transitions of care through outreach following emergency department visits, hospitalizations, and crisis encounters.
- Addresses barriers to care including transportation, housing instability, food insecurity, medication access, and insurance coverage.
- Maintains readiness of the Mobile Health Unit including supplies, equipment, infection prevention standards, and inventory management.
- Participates in community outreach events, health fairs, and engagement activities.
- Supports quality improvement activities and mobile health performance measures.
- In case of patient referral to the Emergency Department or another facility, coordinate care with appropriate staff at the other facility.
- Support the development of medication and healthcare education for patients.
- Support the mobile health team and coordinates with other facilities and providers.
- Assist with mobile unit setup, breakdown, and daily operational readiness.
- Ensure vaccine storage, emergency supplies, and infection control standards are maintained.
- Support scheduling, registration, patient flow, and outreach activities.

Qualifications
1. Education: ADN in nursing. BSN Preferred
2. Job Specific Requirement - Minimum two years of nursing experience in primary care, behavioral health, community health, urgent care, emergency medicine, hospital nursing, integrated care, or mobile health preferred.
3. Ability to work independently in community-based settings and adapt to changing environments.
4. Job Specific Requirement - Registered nurse with the Arizona State Board of Nursing
5. BLS required
6. Must pass a TB Test, Drug Screen and Background Check
7. Knowledge, Skills, & Abilities: Interpersonal skills which allow for professional and positive relationships with co-workers, members, families, and other community members.
8. Knowledge, Skills, & Abilities: Good communication skills, evidenced by a non-judgmental approach, effective listening and positive interactions.
9. Knowledge, Skills, & Abilities: Strong understanding of substance use, addiction and recovery principles, primary care, and preventive health. Knowledge of chronic conditions experienced by those with serious mental illness preferred.
10. Computer Skills: Microsoft Word, Excel, PowerPoint, Outlook, electronic medical record, NextGen preferred.
11. Must have valid Arizona drivers license, be 21 years of age with minimum 4 years driving experience, and meet requirements of Terros Healths driving policy.
12. Must have a valid Arizona Fingerprint Clearance card or apply for an Arizona fingerprint clearance card (Level 1) within 7 working days of assuming role.
13. Preferred: Experience in mobile health, FQHC, public health, population health, or outreach programs.
14. Preferred: Experience serving vulnerable, homeless, uninsured, or underserved populations.
15. Knowledge of social determinants of health, health equity, chronic disease management, and preventive health services.
