## Responsibilities
The Manager of Chronic Care Management (CCM) provides clinical and operational leadership for chronic care management and population health programs across Community Health System. This role leads multidisciplinary teams responsible for coordinating care for high-risk and rising-risk populations to improve clinical outcomes, patient engagement, quality performance, appropriate utilization, and continuity of care. The Manager develops and optimizes standardized care management models and partners with physicians, clinical operations, population health, analytics, and other key stakeholders to integrate care management into clinical workflows and advance value-based care initiatives. This position ensures programs meet applicable regulatory, payer, documentation, billing, and quality requirements while using performance data to drive continuous improvement.

## Qualifications
Educationo Bachelor's Degree in Nursing requiredo Master's Degree in Nursing, Healthcare Administration, Business Administration, Public Health, or related field preferred Experienceo Five (5) years of clinical nursing experience required.o Three (3) years of experience in care management, case management, population health, chronic disease management, ambulatory care, or a related area required. Licenses and Certificationso Current and valid Registered Nurse license to work within the state of California requiredo Current Basic Life Support (BLS) for Healthcare Providers by American Heart Association (AHA) required
