Our MDS Coordinators:
- Manage and coordinates the required resident assessment data collection process.
- Assess the status of the MDS forms for all current residents/patients. Establishes a schedule of known assessments to be done during the year with the Director of Nursing Services.
- Review results of last DSHS Survey to determine areas of needed improvement in the MDS process.
- Start and completes nursing section of MDS assessment. Ensures assessments are completed by other disciplines in a timely manner. Transmits completed MDS required materials to the State via computer.
- Maintain knowledge of applicable laws, policies, and procedures regarding RAI/MDS.
- Consult with residents, families, and staff. Communicates resident information clearly and effectively to staff and management. Interviews residents and families as needed or required for information needed on MDS assessment and to help establish plan of care.
- Participate in the admission, Medicare, and quarterly care conferences as needed.
- Assist in resident care and evaluations as needed.
- Perform other duties as assigned.
What you need to succeed!
- Bachelor’s degree in related health discipline, or the equivalent combination of education and experience.
- Current State Licensing as an LPN or Nurse with a minimum of two years of experience with long term care population
- Willing to work as a member of a team in the Health Center and work under the direction of the Director of Nursing Excellent organizational & management skills.
- Must be able to effectively communicate in English (written, verbal)
- First Aid / CPR Certification
- Must be able to complete a background check and other pre-employment screenings, which may include a Physical, TB, Drug Screen per state/local guidelines.
