Licensed Practical Nurse (LPN) Home Health/Clinic - 12 Week Contract
We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) for a 12-week contract to deliver high-quality care to patients. This role involves providing direct patient care in both clinic and home health settings under the direction of the Registered Nurse and interdisciplinary team. The LPN will assist with clinical care coordination, monitor patient conditions, and support the delivery of patient-centered care while maintaining the highest standards of nursing practice.
As a Licensed Practical Nurse, here's a big picture view of what you'll do:
- Works collaboratively with the interdisciplinary team (IDT) to implement and support comprehensive plans of care for participants.
- Provides skilled nursing care and treatments within the scope of LPN practice and under the supervision of a Registered Nurse as required.
- Develops and maintains positive relationships and communication with co-workers, participants and their families/significant others, and members of the community.
- Participates in interdisciplinary team meetings and communicates relevant participant updates to the team.
- Assists the interdisciplinary team in addressing participants' nursing, self-care, and functional needs related to their health conditions.
- Monitors participant health status and promptly reports changes in condition to the appropriate healthcare provider or supervising RN.
- Assists with care coordination activities and supports participant care planning and implementation.
- Participates in utilization review activities, quality improvement projects, and team meetings as assigned.
- Provides oversight and guidance to Home Care Aides and CNAs in accordance with state regulations and organizational policies.
- Participates in family meetings, staff meetings, in-services, training, and orientation programs as required.
- Follows all Program Policies and Procedures and Occupational Safety and Health Administration (OSHA) safety guidelines.
- Protects privacy and maintains confidentiality of all company procedures and information about employees, participants, and families.
- Practices standard precautions and follows Program Infection Control protocols.
- Performs other duties as required or requested.

A few must-haves for our Licensed Practical Nurse:
- Graduate of an accredited Practical Nursing program required.
- Current LPN License required - South Carolina License/Compact License in good standing.
- Minimum of 1 year of nursing experience required; home health, long-term care, geriatrics, or managed care experience preferred.
- Experience working with a frail or elderly population preferred.
- Strong clinical assessment, communication, and documentation skills.
- Must provide a valid driver's license and proof of auto insurance.
- Current CPR/BLS certification required.
- Ability to pass drug screening and background checks.
- Up-to-date required immunizations (Tdap, MMR, Varicella, Flu) and physical exam or statement of good health.
- Must meet all applicable health and physical requirements necessary to safely provide patient care.
