This is a highly responsible and sensitive position that provides case management to clients in the Ryan White Program.

This position provides comprehensive medical and non-medical case management services to clients living with HIV/AIDS.

This position serves as part of the interdisciplinary FDOH - SJC Early Intervention Services (EIS)/Ryan White Program Team.

These activities include but are not limited to:

- Interviewing clients while completing

Initial and ongoing risk, need, and social assessments,

- All required forms,

- Eligibility process

- Evaluating clients’, including medical, social and psychological aspects, and developing an individualized Plan of Care (POC)

- Coordinating and collaborating on going-care, education, and support with EIS Program Team to meet the individual POC

- Coordinating services by connecting clients with community resources, healthcare providers, rehabilitation programs, or legal services, ensuring continuity of care and timely access to necessary support and services

- Advocating for clients by helping them navigate complex systems, communicate with professionals, and make informed decisions about their care and/or other matters

- Maintaining regular communication with clients and families, track progress, update care plans, and adjust strategies as needed to achieve desired outcomes

- Keeping detailed records of client interactions, treatment plans, referrals, and outcomes, and may prepare reports for supervisor

- Maintaining information in a confidential manner and disclosing information only after proper authorization has been obtained

- Providing education, counseling and/or making appropriate referrals and linkages to meet client needs

- Maintaining individual case files for enrolled clients that includes documentation of all case work activities and service utilization in required systems

- Closing or transferring files as appropriate for client activities

- Providing ongoing review of and eligibility for health and/or social services

- Assisting with the coordination of all HIV counseling and testing

- Completing HIV testing and counseling in accordance with statutory requirements

- Conducting home visits and participating in outreach events

Assists with programmatic activities which include but are not limited to:

- Compiling, maintaining and preparing data on clients involved in the program

- Providing necessary reports and data for grant submission/application and/or administrative reporting

- Provides data as needed to supervisor to assist in preparing grant updates/applications

Performs other duties as required.

Required Knowledge, Skills, and Abilities:

Knowledge, skills and abilities, including utilization of equipment, required for the position:

- Knowledge of basic case management concepts and processes

- Knowledge of interviewing techniques

- Knowledge of HIV/AIDS treatment and care

- Skilled in written and oral communication

- Skilled in active listening, empathy, and conflict resolution

- Ability to work independently but also work as part of a team when needed

- Ability to communicate proficiently and effectively with clients and professionals

- Ability to utilize MS Office programs and database management systems

- Ability to complete data/statistical analysis and scientific methods

- Ability to prioritize and organize work assignments

- Ability to operate technical equipment

- Ability to work in a fast-paced environment

- AIDS Education and Training Center (AETC) sponsored case management training completed within six months of hire

Licensure/registration/certification requirements (If applicable, list the appropriate Florida Statute or federal regulation cite): Valid Florida driver license, HIV 500/501 certification within 60 days of hire (Florida Statute 381.004)

Other job-related requirements for this position: Incumbent may be required to work before, during and/or beyond normal work hours or days in the event of an emergency. Emergency duty required of the incumbent includes working in special needs or Red Cross shelters, or performing other emergency duties including, but not limited to, responses to or threats involving any disaster or threat of disaster, man-made or natural.

Working hours: (A) Daily from 8:00 to 5:00 (B) Total hours in workweek 40 (C) Explain any variation in work (split shift, rotation, etc.) This position requires daily in-office or field work.

Qualifications:

Minimum - Must have a valid Florida driver's license. Must be willing to physically come in the office daily and or perform field work.
