What You’ll Do
The Patient Navigator II functions within the clinic as a co-coordinator, advocate, interpreter, and educator for surgical candidates requiring medical clearance and optimization. The Patient Navigator II plays an integral part for ensuring patients undergoing complex surgery have the opportunity to follow a best practice pathway pre and post-procedure and work towards optimal results. They will also be expected to serve as a liaison throughout the facility and in the community regarding services provided for patient optimization and support. The Patient Navigator II will support the Clinic Manager/Certification Coordinator by collecting key information for the dashboard and assisting in developing and tracking all process improvement initiatives.
Minimum Qualifications
- Graduate of an Accredited Program in Practical Nursing
- Licensed Practical Nurse in State of Ohio (LPN), Basic Cardiac Life Support (BCLS)
- Minimum of 5 years experience working in a medical field. Familiarity with one or more years in surgical practice. Experience with musculoskeletal anatomy and/or treatments.
