Person-Centered Planning Coordinator

You'll work closely with individuals with developmental disabilities, their families, service providers, and interdisciplinary teams to develop and coordinate individualized plans that reflect each person's strengths, preferences, needs, and aspirations.

About the Role
The Person-Centered Planning Coordinator develops, coordinates, monitors, and updates individualized service plans. You'll help ensure services are responsive to each individual's goals and delivered in accordance with person-centered planning principles, agency standards, and applicable regulations.
Most importantly, you'll help ensure that the person-not the paperwork-is at the center of the planning process.
What You'll Do
- Develop, coordinate, and maintain individualized, person-centered service plans.
- Partner with individuals, families, service providers, and interdisciplinary teams to identify goals, strengths, preferences, and support needs.
- Monitor service implementation and help ensure supports remain aligned with each individual's goals.
- Coordinate regular plan reviews and make updates as needs, circumstances, and goals evolve.
- Maintain accurate, timely, and compliant documentation.
- Ensure services and documentation meet agency policies and applicable state and federal requirements.
- Advocate for individuals and ensure their voices, choices, and preferences guide the planning process.
- Build strong, collaborative relationships with internal teams, families, providers, and community partners.
- Help identify opportunities that promote independence, inclusion, community participation, and meaningful outcomes.
What We're Looking For
Required Qualifications
- Bachelor's degree in Human Services or a related field.
- At least one year of experience supporting individuals with developmental disabilities.
- Valid New York State driver's license and a driving record that meets agency insurance requirements.

- Excellent written and verbal communication.
- Experience coordinating services and maintaining detailed documentation.
- Ability to build positive working relationships with individuals, families, and multidisciplinary teams.
- Understanding of and commitment to person-centered planning, inclusion, and advocacy.
- Ability to manage multiple priorities while maintaining accuracy and attention to detail.
