Outreach Case Manager

Summary/Objective

Under the supervision of the SOP Director, the Outreach Case Manager will provide intensive case management, including specialized trauma-informed and evidence-based interventions with youth. The individual in this position will create safe and welcoming environments for youth being referred to and enrolled in program services, with a focus on creating a positive relationship to engage and retain youth in program services. This individual is responsible for implementing evidence-based interventions with fidelity, providing skill development opportunities for youth, and providing services in non-traditional settings.

Essential Functions

- Conduct outreach, adhering to the program schedule, in order to facilitate the provision of concrete resources to youth.

- Develop and maintain an extensive knowledge of resources in the community.

- Refers, advocates, and assists youth in accessing resources that support their goals within their community.

- Coordinates and/or participates in collaboration with community resource providers.

- Create a caring, safe, and healing environment that keeps the youth at the center of care throughout their experience with the program.

- Assist youth with completing intake/enrollment/assessment and discharge/follow-up forms and required agency and grant paperwork.

- Work with youth to determine the most appropriate location to complete the enrollment paperwork (home, community, and/or Crittenton Center), including data collection instruments and other screening and assessment tools.

- Define strategies for communicating with the youth (face-to-face location in community, phone, email, text, and/or warm hand offs at an office location with other referral agencies) to engage the youth and begin to build a positive relationship.

- Maintain a caseload and meet with youth weekly in the community at the location of their choice, or as appropriate.

- Develop positive relationships with community organizations to promote SOP services and connect youth with needed and available community resources.

- Provide evidence-based interventions as appropriate and with fidelity.

- Provide intensive case management services to assist in navigating other service systems to secure resources such as housing, transportation, food, and other practical items to aid families in their healing and recovery.

- Follow-up with clients and agencies as appropriate for their needs.

- Participate in training, community events, and other professional development activities as assigned.

- Facilitate groups with youth for skill development, maintaining fidelity to EBPs and documentation as appropriate.

- Support youth in shelter programming through structured, skill-focused activities and interactions.

- Complete all required documentation, assessments, and data collection as required.

- Ensures all documentation and data collection is completed in the time frames set by program procedures and requirements.

- Actively participates in weekly staff meetings with the Program Director and other program staff.

- Transports resources or provides transportation for clients.

- Maintains appropriate training for role, including EBPs, CPI, CPR/First Aid, and other training courses as applicable.

- Maintains high standards of confidentiality of all client records and information, utilizing the agencys Electronic Health Record System.

- Adhere to the Social Work Code of Ethics and Crittenton Center code of ethics.

- All other duties as assigned.

Supervisory Responsibility

This position does not have supervisory responsibilities.

Work Environment

This position operates in both office and community-based settings. The role involves collaboration with agency staff, community partners, and families, and may include virtual or home visits.

Physical Demands

The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. While performing the duties of this job, the employee is regularly required to talk, hear, and use standard office equipment. The employee must transport resources or provide transportation for clients.

Position Type/Expected Hours of Work

This is a full-time position. Typical hours are Monday through Friday from 8:00 a.m. to 5:00 p.m.; may include some nights or weekends.

Travel

Travel required during the business day.

Required Education and Experience

- Minimum of a Bachelors Degree in social work or other related field and experience in program development.

- Computer Knowledge and Resource Navigation Experience.

- Excellent communication skills both verbal and written; excellent organizational and interpersonal skills; detail oriented.

Certificates, Licenses, Registration

Valid Iowa Drivers License.
