Summary:

As a Community Health Worker serving the Ruidoso area, you'll play a vital role in improving the health and well-being of perinatal individuals, children, and families throughout the community. Based at Lincoln County Medical Center, you'll help connect patients with essential local resources, provide health education, guide individuals through the healthcare system, and co-lead an innovative produce prescription program that promotes healthier lifestyles and better outcomes.

Responsibilities:

- Collaborate with the Care Manager to help implement individualized care plans that support patients' health and wellness goals.
- Act as a trusted resource for patients and families by reinforcing health education, answering questions, and encouraging engagement in their care.
- Communicate patient-reported changes, concerns, or barriers to the Care Manager and/or Primary Care Provider to support timely interventions.
- Foster a safe, welcoming, and supportive environment for patients, families, and visitors.
- Connect individuals and families to community resources and services that promote improved health outcomes and quality of life.
- Stay current on Community Health Worker best practices, healthcare trends, and Presbyterian Healthcare Services' programs and mission to ensure high-quality service.
- Pursue ongoing professional development through educational programs, workshops, in-service training, and review of current industry literature.
- Build positive relationships with patients, families, healthcare team members, and community partners through effective written and verbal communication.
- Represent the organization by participating in local, regional, and state Community Health Worker meetings, events, and collaborative initiatives.
- Perform additional duties and responsibilities as assigned in support of departmental and organizational goals.

Qualifications:

- High School Diploma or GED required.
- Additional specialized short-term training in clinical, healthcare, or medical-related skills is preferred.
- Ability to communicate effectively in the language(s) spoken by the population served is required.
