Position Summary

The Transition Coordinator reports to the Manager, Infection Prevention and Control and Clinical Collaborative Practice (IPAC & CCP), and consistent with the Mission, Values and policies of St.Amant, the Transition Coordinator; Pediatrics is a critical role in ensuring the success of St.Amant business and operations.

Exemplifying the mission, vision and values of St.Amant, the Transition Coordinator; Pediatrics works collaboratively with partnered programs while offering employees respectful customer service.

The Transition Coordinator; Pediatrics develops and supports systems of care collaboration, patient flow, and family and staff support between Health and Transition Services, HSC Shared Health Children's Hospital and Community-based care.

The Transition Coordinator; Pediatrics has on-site roles and responsibilities at both locations, including, but not limited to:

- Relationship building and education for care team and leadership staff at Children's Hospital on the Mission, Vision and Values of St.Amant and H&TS
- Active participation with multiple care teams at Children's Hospital to support early identification and care planning for children who will benefit from the available supports in H&TS
- Initial and ongoing family education and support at both locations
- Acts as a resource at both sites within the defined patient population (s)
- Health and Social system navigation to support timely access to resources and discharge planning
- Identification of staff education needs relevant to the population supported at both sites
- Development and maintenance of efficient communication and patient flow pathways between sites

Qualifications

Education

- Graduate of an approved Registered Nursing education program required
- Baccalaureate Degree in Nursing required

Experience

- Minimum of two (2) years recent clinical experience, inclusive of interdisciplinary care planning and complex discharge planning
- Minimum of two (2) years pediatric experience required
- Experience working with individuals with intellectual and developmental disabilities and medical complexity required
- Proficient in computer software applications such as; word processing, spreadsheets, databases and electronic medical records (EMR)
- Experience in Acute Care and experience in health and disability services system navigation is an asset

Other

- Valid Class 5 Manitoba Drivers License and access to own vehicle is required
- Knowledge of and adherence to:

- College of Registered Nurses of Manitoba Standards of Practice for Registered Nurses
- Canadian Nurses Association Code of Ethics for Registered Nurses
- Scope of practice as documented in the Regulated Health Professions Act (RHPA) and Regional/Facility/Unit policies, procedures, protocols and guidelines
- All relevant legislated acts, including but not limited to: Personal Health Information Act (PHIA), Protection of Persons in Care Act, Mental Health Act, Workplace Hazardous Material Information System (WHMIS), Principles of Routine Practices (Universal Precautions)
- Roles and responsibilities of members of the healthcare team
- Principles of delegation and assigning tasks to unregulated healthcare professionals
