Aveanna Healthcare is seeking a Clinical Supervisor – RN to join our team and play a key role in supporting high-quality patient care within the home. This position offers the opportunity to combine your clinical expertise with leadership, education, care coordination, and team development.

As a Clinical Supervisor, you will work closely with patients, families, caregivers, and an interdisciplinary team to ensure patients receive safe, compassionate, and individualized care. You’ll also have the opportunity to mentor and develop clinical caregivers while helping drive quality and positive patient outcomes.

What You’ll Do

- Coordinate and oversee care for assigned patients from admission through discharge, developing individualized, goal-directed care plans.

- Conduct supervisory visits in the home to assess patients, families, the care environment, and clinical caregivers.

- Provide ongoing clinical guidance, support, education, orientation, and feedback to caregivers.

- Build strong relationships with patients, families, caregivers, and members of the interdisciplinary team.

- Assess patients and provide direct patient care when needed.

- Review clinical documentation and medical records to promote accuracy, compliance, and quality.

- Monitor clinical trends, incidents, and performance data to identify opportunities to improve patient outcomes and reduce risk.

- Support quality improvement initiatives, patient safety efforts, and CHAP standards.

- Provide clinical updates and assist with re-authorization to support continuity of care.

- Help ensure patients have the appropriate equipment, supplies, and resources needed for safe care.

- Contribute to caregiver retention by creating a supportive, engaging environment and helping team members feel confident and successful in their roles.

- Participate in hiring, onboarding, performance evaluation, and other employment decisions involving clinical caregivers, as appropriate.

- Collaborate with leadership and the interdisciplinary team to resolve concerns and support effective care coordination.

- Participate in an on-call rotation and provide timely support when patient or caregiver needs arise.

- Promote a culture of clinical excellence, safety, professionalism, and compassionate patient care.

- Follow all applicable company policies, state and federal regulations, and professional nursing standards.

What You’ll Bring

- Diploma, Associate, or Bachelor’s degree in Nursing from an accredited RN program.

- Current RN license in the applicable state(s).

- Clinical experience as required by applicable state and federal regulations.

- Valid driver’s license and acceptable motor vehicle record.

- Ability to travel throughout the service area.

Preferred Qualifications

- RN experience in home care, private duty nursing, or a related clinical setting.

- Experience supporting, mentoring, or supervising clinical caregivers.

- Knowledge of private duty nursing and/or home-based care.

- Strong clinical judgment and the ability to make confident decisions in changing situations.

Physical & Environmental Requirements

- Must be able to speak, write, read, and understand English.

- Must be able to travel throughout the assigned service area; company vehicles or transportation are not provided.

- Ability to occasionally lift, carry, push, or pull up to 50 pounds.

- Ability to walk, stand, bend, kneel, reach, twist, sit, and climb stairs as required.

- Must have adequate visual and hearing acuity.

- Must be able to safely reposition patients and move equipment as needed.

- Must be able to appropriately respond physically and mentally to emergency situations in the home or during transport.

- Position may involve exposure to allergens, blood, bodily fluids, infectious diseases, and other environmental conditions commonly associated with home healthcare.
