Job Summary
Plans and coordinates intake and referrals for Hendrick Home Health; ensuring the timely receipt and review of accurate patient information; physician orders; and supporting documentation to facilitate appropriate admissions and quality patient care.
Job Requirements
M i n i m u m E d u c a t i o n
- LVN
M i n i m u m W or k E x p e r i e n c e
- 1 to 3 years of experience as an LVN; preferably with experience in home health.
R e q u i r e d L i c e n s e s/ C e r t i f i c a t i o n s
- CPR (BLS)
R e q u i r e d S k ill s ; K n o w l e dg e ; a n d A b ili t i e s
- Strong communication skills; verbal and written alike.
- Understanding of physician orders and clinical definitions.
- Knowledge of patient retention; ability to differentiate levels of urgency; and organizational skills.
- Knowledge of regulatory standards regarding physician offices and home health; specifically in compliance with Medicare.
