**REMOTE POSITION FOLLOWING INITIAL HYBRID TRAINING | ROTATING DAYS/NIGHTS, WEEKENDS AND HOLIDAYS. REQUIRES HIGHLY MOTIVATED INDIVIDUAL**
The Transplant RN Care Coordinator reports to the Transplant Programs Manager within the Transplant Center. The Transplant RN Care Coordinator plays a critical role in supporting transplant patient care and program operations. The Transplant RN Care Coordinator applies advanced clinical judgment, consultation, education, and evidence-based practice to promote high-quality care for transplant patient populations.
This position requires a highly motivated, adaptable RN who is comfortable working independently, managing competing priorities, and responding effectively in time-sensitive situations. Responsibilities also include supporting the training and development of new team members and maintaining current knowledge of applicable UNOS/OPTN and CMS regulatory requirements. This position is responsible for the ongoing training of new staff and for keeping up to date with regulatory changes in UNOS and CMS.
Training and On-Site Requirements
Training for this position requires a minimum of 3 months and follows a hybrid model, combining on-site training at the downtown Charleston campus with remote training alongside an assigned preceptor. Following completion of initial training, the position transitions to a remote work model; however, team members must remain flexible and available to attend monthly in-person meetings in Charleston and to participate in additional on-site training, meetings, or departmental activities as needed.

Scheduled Work Hours/Shift:
Three 12-hour rotating shifts per week, including days, nights, weekends and holidays.
Day shift: 7:00am -7:30pm
Night shift: 7:00pm-7:30am

Job Summary/Purpose: The Transplant RN Care Coordinator reports to the Transplant Programs Manager within the Transplant Center. Under general supervision, the Transplant RN Care Coordinator utilizes advanced clinical management, consultation, education, and evidence-based practice to support safe, high-quality care for transplant patient populations.
The Transplant RN Care Coordinator collaborates with members of the multidisciplinary transplant team, applies sound clinical judgment in a dynamic environment, and effectively prioritizes responsibilities based on patient and program needs. The position also supports the orientation and ongoing training of new staff and maintains current knowledge of applicable UNOS/OPTN and CMS regulatory requirements.
Minimum Training and Education: A Bachelor of Science in Nursing (BSN) and a minimum three years of registered nursing experience are required. Previous transplant nursing experience preferred.
Required Licensure, Certifications, Registrations:
Current South Carolina RN licensure or a qualifying multistate/compact state RN license required. Current Basic Life Support (BLS) is required through the American Heart Association (AHA), an AHA-recognized equivalent, or the American Red Cross CPR/AED for Professional Rescuers and Healthcare Providers. Certified Clinical Transplant Coordinator (CCTC) certification is preferred.

2. Proficiency in manual dexterity and fine motor skills for tasks such as taking patient vitals, administering injections, or conducting physical exams.
3. Capacity to lift and move patients or medical equipment as needed.
4. Visual acuity and color perception to accurately read medical charts, documents, and monitors.
5. Auditory ability to effectively communicate with patients and colleagues, as well as to perceive important sounds such as alarms or patient instructions.
6. Stamina and resilience to handle the physical and emotional demands of providing patient care, including long shifts and potentially stressful situations.
7. Mobility to navigate clinical environments, including moving between patient rooms, offices, and other areas of the facility.
8. Compliance with infection control protocols and proper use of personal protective equipment to maintain a safe and healthy work environment.
