Employment Type: Temporary Full-Time (temporary backfill)

Type: Union

Hours: Monday to Friday, with flexibility for occasional evening or weekend coverage

Rate of Pay: $31.37 per hour

PCN: 02-300-010

Position Summary

The Housing Case Manager I is responsible for supporting Guests in a range of housing services including housing searches, moving plans, financial resources, housing support and eviction prevention.  Reporting to the Manager of Guest Services, the Housing Case Manager is responsible for:

Duties/Responsibilities

- Engaging with individuals experiencing homelessness, as well as barriers to services/housing to identify appropriate support.

- Actively working to remove or lower barriers for guests seeking services, using a harm reduction approach.

- Developing housing goals with Guests and assisting with landlord mediation, housing searches, funding requirements etc.

- Supporting Guests through outreach visits for the purpose of viewings, meetings with landlords and accompanying to relevant appointments.

- Providing support to Guests in their homes with respect to the development of life skills and community integration.

- Navigating systems alongside guests, taking on an advocacy role as required.

- Providing ongoing housing support to assist Guests in maintaining their housing through outreach and assertive engagement.

- Documenting Guest interactions on a shared electronic system in a way that strengthens collaboration across the organization.

- Building and maintaining relationships with Guests, while maintaining clear and consistent boundaries.

- Working as a collaborative member of TGP’s clinical and guest services team, contributing to a continuum of care service provision for guests. Participates as part of a larger multi-disciplinary team of co-located service providers.

- Engaging in de-escalation, crisis and safety planning with guests who may exhibit behaviours that present as barriers to service.

- Submitting required data and reports to the Manager in a timely manner.

Qualifications and Skills

- Bachelors Degree or diploma in health/social services

- Minimum of two years of relevant experience

- Knowledge of available and relevant community resources for marginalized persons

- Sound relationship-building abilities and skills in individual support, conflict resolution, and empathetic listening

- Demonstrate understanding and sensitivity to links between diversity and marginalization.

- Strong interpersonal skills, ability to creatively build relationships with those who may be socially isolated.

- Proficient and professional documentation

- Demonstrate skills in systems navigation.

- Creative, adaptable problem-solving skills

- Clear Record of Conduct and Vulnerable Sector Check

Equivalencies in education and experience will be considered.
