About this role

Job Summary

The Resident Liaison connects residents with the community resources and support services they need to live safely and independently. Acting as a bridge between residents, property management, local service agencies, and the Owner, the Resident Liaison builds relationships with community providers, helps residents navigate available services, coordinates resident activities and communication, and reports on service delivery to the Owner and Director of Social Services. This role supports Evergreens commitment to resident well-being and quality of life at the property.

Key Responsibilities

Community Partnerships & Resource Navigation

- Establish and maintain relationships with local service providers.

- Develop liaisons with community agencies to help link residents to needed services.

- Assist residents in navigating the local service provider and agency network.

- If a resident requires a level of care not available through community services, assist the resident in identifying other options.

- Obtain needed items for residents through community agencies, and assist residents in securing needed items in their homes.

Resident Education & Engagement

- Create and educate residents on available services by sharing the Community Resource Manual.

- Work with ERES to set up educational and other relevant programs for residents.

- Form an activity committee among residents and interested parties, and participate in overall planning for property activities.

- Create and distribute a monthly property newsletter.

Reporting, Confidentiality & Compliance

- Provide quarterly updates on services provided to the Owner and Director of Social Services, including a calendar of events and resident attendance.

- Report suspected abuse or neglect to the proper agencies and take proactive steps to protect residents.

- Maintain resident files in a secure area to ensure confidentiality.

- Demonstrate sensitivity and responsiveness to cultural differences within the resident population, and maintain resident confidentiality at all times.

Additional Services (case-by-case, with Owner consent)

- Provide training to property management staff on identifying and working with specific types of physical or mental disabling conditions, applicable fair housing rules, etc.

- With resident approval, conduct an in-depth assessment and create a Case Management Plan.

- Cooperate in appropriate legal proceedings when necessary, and coordinate with hospital staff regarding discharge planning for residents returning to their apartments.

- Identify education and health programs to enhance residents quality of life.

Education & Experience

- Bachelors or Associates degree in Social Work, Counseling, or another social science field is desired.

- Previous experience as a Social Service Coordinator or Resident Liaison is preferred.

Technical & Physical Skills

- Working understanding of social service funding, eligibility requirements, and access to community services.

- Familiarity with the propertys neighborhood and the services available to residents there.

- Strong oral communication skills: speaks clearly and persuasively, listens and seeks clarification, responds well to questions, and is comfortable presenting to groups and participating in meetings.

- Strong written communication skills: writes clearly and informatively, edits for spelling and grammar, varies style to fit the audience, and presents numerical data effectively.

Other Requirements

- Must be able to effectively communicate with a diverse population and range of personalities.

- Access to reliable transportation to travel between the property, resident homes, and community agencies.

- Must be capable of adapting, with minimal or no advance notice, to changes in how business is conducted and work is accomplished, with no diminishment in work performance.

Work Environment & Physical Demands

This role is based primarily on-site at the property, with regular local travel to community agencies, service providers, and occasionally resident homes or hospitals to coordinate care. The role involves frequent walking, standing, and interaction with residents and outside partners, as well as extended periods of sitting for administrative and reporting work.
