POSITION SUMMARY:
The Family Support & Engagement Specialist Supervisor is responsible for coordination of the subcontracted visitation component at Kids in Distress of the Healthy Families Broward Project. This position functions as the field supervisor for the family support & engagement specialists in the program and reports to the Senior Director of Prevention Programs at Kids in Distress and will also function under the direction and guidance of the Healthy Families Broward Program Manager.
PRIMARY DUTIES & RESPONSIBILITIES:
- -Provides clinical, administrative and reflective supervision with family support & engagement specialists as per HFF Policy and Procedures
- -Keeps a log of daily supervisory notes which documents all problems, concerns, feedback, and accomplishments regarding assigned staff.
- -Reviews documentation, including home visitation records, progress notes, Ages and Stage Questionnaires, Family Goals, etc. Submits required revisions back to staff within 48 hours.
- -Maintains a detailed supervision binder for each family support & engagement specialists with case review notes for each active participant.
- -Completes at least two home visit shadowing per year for each family support & engagement specialists and additional ones for newly hired staff. Supports assigned family support & engagement specialists with visitations, Level "CO" staffing, and case conferences when appropriate.
- -Completes probationary and annual evaluations on all assigned staff.
- -Completes evaluations at least two weeks after the required due date.
- -Monitors employee individual goals at least one time per month.
- -Ensures that each assigned staff has maintained their required amount of training.
- -Audits each assigned staff's training file quarterly to track compliance.
- -Responsible for the assignment and maintenance of referrals to assigned family support & engagement specialists.
- -Ensures that each family support & engagement specialist's case weight is maximized to ensure that the program serves 120 participants per year.
- -Ensures that the case weights for individual family support & engagement specialists does not exceed the contractually required case weight of 30.
- -Reviews cases on a routine and consistent basis for level changes.
- -Provides support to the family support & engagement specialists during leaves of absences.
- -Provides necessary coverage of home visits and/or contact with assigned families during staff absences as necessary.
- -Provides support to the family support & engagement specialists with a positive, motivating manner 100% of the time.
- -Provides monthly prompt lists to the family support & engagement specialists in regards to due dates on contractually required outcome documents including immunization schedules, well baby check-ups, ASQ's, family goals, etc.
- -Addresses complaints and/or grievances that arise from both staff and participants in a diplomatic, objective manner 100% of the time.
- -Provides 24-hour on-call availability to family support & engagement specialists in the event of crisis with assigned families in the program.
- -Reviews and approves employee timesheets, enters data into the agency TC1 time reporting system, approves leave requests, and monitors time and attendance of assigned family support & engagement specialists.
- -Interviews prospective candidates during program vacancies and submits top candidates names and documentation to the Prevention Program Senior Director.
- -Responsible for ensuring that all newly hired family support & engagement specialists are trained in the Prevention Program orientation, and day to day procedures.
- -Completes probationary reviews after 90 days of employment for each family support & engagement specialists.
- -Completes an annual performance review on each assigned family support & engagement specialists which includes performance level attainment in each responsibility, a narrative evaluation, and goal formulation.
- -In partnership with the staff member, formulates employee goals that are consistent with the program and agency strategic plan.
- -Reviews these goals with the employee for attainment and evaluation at least once monthly.
- -Obtains referrals from the Healthy Families Broward Assessment Supervisor, documents the referral, completes the needs and concerns checklist and assign cases to the family support & engagement specialists in a timely manner.
- -Ensures that family support & engagement specialists make initial contact with newly assigned referrals, within 30 days of assignment of the referral.
- -Responsible for the oversight of data into the Healthy Families Florida database.
- -Performs consistent cross-audits of system data and file documentation to ensure 100% accuracy.
- -Responsible for ensuring that a TANF form is completed for each participant in the program on an annual basis.
- -Ensures that family support & engagement specialists maintain the outcome of at least 80% of families will receive at least 75% of home visits as prescribed by the leveling system.
- -Ensures that the program retention rate is above 69% annually.
- -Ensures that family support & engagement specialists maintain the outcome of at least 90% of participants will have a family goal developed within 90 days of program enrollment and then updated every 90 days thereafter.
- -Ensures that family support & engagement specialists maintain the outcome of at least 90% of target children will receive and ASQ developmental screen within five months of the child's date of birth and then at prescribed intervals thereafter.
- -Ensures that family support & engagement specialists maintain the outcomes contractually required for participant up-to-date immunization and well-baby checks.
- -Submits programmatic quarterly reports on agency reporting tool by the 10th day following each quarter. (Family Intervention only).
- -Compiles the Healthy Families Broward monthly client report to the KID Program Senior Director and Healthy Families Program Manager by the 10th day of each month.
- -Audits one open file per family support & engagement specialists' staff member on a monthly basis, and audits 100% of files closed, and reports the results on the KID quarterly report.
- -In collaboration with Healthy Families Broward and with assistance from the family support & engagement specialists, plans and conducts at least 1 participant event per quarter.
- -Assists in the planning of participant support groups and also assists in and facilitation of support related activities.
- -Takes an active role in the development and implementation of the Healthy Families Plus Program which includes couples group work and the active involvement of fathers in the program.
- -Attends Healthy Families Broward group supervisor supervision with the HFB collaborative partners.
QUALIFICATIONS :
- Bachelor's degree with coursework in areas related to social services, child development, abuse and neglect issues and minimum three years of related work experience in the diverse areas of the Social Work field.
- Experience in child abuse prevention, family preservation, and case management; or equivalent combination of education, training and work experience.
- Knowledge, training or advanced course work in human development, social work, or related field(s) and experience in Broward County Child Welfare System preferred.
- Satisfactory local and level II background screening .
- Valid State of Florida Driver's License with appropriate insurance coverage.
- Access to reliable transportation.
- Access to and use of a cellular phone.
- Must have the ability to read, analyze, and interpret professional journals or technical procedures written in English, the ability to type reports and business correspondence in English, and the ability to effectively communicate in English.
- Excellent writing, computer, organizational and communication skills, experience in the development and utilization of outside community resources, creative planning abilities and the ability to utilize persuasive advocacy and strong, dynamic resources to teach effectively.
- Most possess excellent team building skills, along with the ability to work with subcontracted collaborative partners.
- This position requires a flexible work schedule which includes evening hours in order to provide shadowing and supervision of family support specialists while on home visits with participants in the community.
