Position Summary: As part of the multidisciplinary team, the Nurse will provide healthcare services to students and staff, promote overall wellness, and ensure a safe and healthy school environment. This role involves managing student health needs, responding to emergencies, and supporting physical and mental well-being throughout the school community.

The Nurse will lead the development and ongoing management of a culturally competent, comprehensive, individualized care plan with each individual youth. Assess individuals/families with behavioral health and/or substance use needs and provide treatment and referral services. Provides medical case management oversight and consultation for South County Academy programs. Assures compliance for agency and state/federal medication supply storage and disposal procedures for assigned programs. The Nurse will also prepare and execute student group curriculum which promotes healthy lifestyles and public health education. Provides emergency crisis interventions and assessments as needed. Provide clinical treatment for clients as needed.

Specific Duties/Responsibilities:

- Provide first aid and emergency care for students and staff

- Assess and manage student health concerns, illnesses, and injuries

- Administer medications and monitor students with chronic conditions (e.g., asthma, diabetes)

- Maintain accurate and confidential health records

- Develop and implement individualized care plans and emergency action plans

- Conduct health screenings (vision, hearing, etc.)

- Educate students and staff on health, hygiene, and disease prevention

- Monitor immunization compliance according to state regulations

- Collaborate with parents, teachers, behavioral health staff and healthcare providers

- Administration of medication to students as prescribed on a daily basis

- Coordination and compilation of student health records and screenings

- Ensuring people have access to primary care services and ongoing periodic laboratory testing

- Conducting primary care screening and monitoring of key health indicators and health risk

- Monitoring status indicators and changes in the status of chronic health conditions

- Promoting a healthy behavior lifestyle and establishing educational materials for the daily education of the students including but not limited to peer public health education groups

- Providing consultation to South County Academy residential staff members, youth and families: Conducting consultation in ways that compliment and support the learning style of the person you are coaching, as defined by the person you are coaching

- Actively participating in weekly team meetings

- Works collaboratively across department and divisions to ensure quality service

- Obtains necessary consents and release of information for care coordination and treatment planning activities

- Completes internal and/or external referrals to link people with the appropriate services and resources

- Provides nursing services and primary care assessment and monitoring using evidence-based clinical practices and interventions, that are consistent with the persons values, preferences, and individualized needs

- Encourage positive environment and behavioral support by utilizing a restorative justice framework and principles

- Work as part of a team to support the overall functioning of the program and ensure that all Policies and Procedures are followed

- Become trained, competent and able to perform emergency de-escalation and support strategies when applicable

- Teach client/family self-advocacy skills

- Build and maintain positive, working relationships with community organizations and service providers

- Maintain confidentiality of client/family

- Provide sensitivity to client/family cultural, ethnic, racial and socioeconomic issues and diversity

- Serve as a positive role model to client and colleagues

- Complete all professional and administrative documentation and forms in accordance with agency standards and within established guidelines and timeframes

- Maintains compliance with FSRI policies, including those relating to HIPAA and all state and federal agencies, including DCYF

- Proficient in using an electronic health record as well as project-specific software

- Participates in all meetings as required by the team, supervisors and that are necessary for the role both internally and externally

- Attend and participate in all required program and agency trainings

- Provide on-call coverage/support in conjunction with other members of the management team.

- Approaches all work through a trauma and diversity and inclusion lens

- Performs additional responsibilities consistent with Agency needs, defined in consultation with supervisor(s)

Qualifications:

- Bachelors degree in nursing (BSN) and a RN licensed in the state of RI

- Experience working in a residential healthcare setting, preferably in care management

- Experience with children, adolescents and adults from diverse social, cultural and ethnic backgrounds

- Skilled in operating various medical record software and hardware, word-processing, and database software programs

- Spanish Speaking preferred and is compensated

- Excellent multitasking and communication skills a must

- Ability to work independently and part of a team

Physical Requirements: This position requires community visits, employees in this position must have the ability to:

- Current drivers license, reliable transportation, registration and auto insurance

- Ability to communicate effectively

- Travel to and from, community locations and office site, which could include using walkways, stairs and/or elevators

- Ability to lift up to 20lbs

- Provide emergency coverage; ratio to the programs as needed.

- Must be able to work remotely and in person adhering to PHI requirements
