About this role
The Maternal Health Care Partner supports pregnant, postpartum and parenting patients by connecting them with healthcare, social services and community resources. The role provides culturally responsive navigation, education and follow-up while strengthening partnerships that support maternal, prenatal, postpartum and parenting needs in rural communities.
ESSENTIAL FUNCTIONS:
- Identify, assess and maintain an up-to-date map of community resources supporting maternal, prenatal, postpartum, infant and parenting needs.
- Screen patients and community members for health-related social needs and identify barriers that may affect maternal health, access to care or family stability.
- Provide navigation and follow-up support to pregnant, postpartum and parenting patients seeking healthcare services, community programs and social support resources.
- Help patients understand available services and support coordinated connections to prenatal care, postpartum care, primary care and other appropriate programs.
- Develop and maintain collaborative relationships with community-based organizations and maternal health service providers.
- Document all patient and community encounters, referrals, interventions and outcomes accurately and timely in eClinicalWorks and other designated databases.
- Support required data tracking and documentation for program reporting and applicable billing activities.
- Participate in community events and represent Sun River Health in maternal health outreach, education and engagement activities.
- Complete required maternal health and Community Health Worker training programs and maintain applicable certifications.
- Participate in case discussions, team meetings, grant-related activities and quality improvement initiatives as assigned.
- Maintain patient confidentiality and comply with organizational policies, grant requirements and applicable regulations.
- Perform other duties as assigned to support maternal and rural health initiatives.
EDUCATION AND CERTIFICATION REQUIREMENTS:
- High school diploma or equivalent required.
- Community Health Worker certification is not required at hire; successful completion and ongoing maintenance of required CHW and maternal health training or certification is a condition of continued employment.
EXPERIENCE REQUIREMENTS:
- Prior experience in maternal health, community outreach, case management, patient navigation, family support or a related service role preferred.
- Demonstrated knowledge of or strong ties to the communities served preferred.
- Bilingual skills preferred based on community needs.
REQUIRED KNOWLEDGE, SKILLS, AND COMPETENCIES:
- Working knowledge of community resources supporting pregnancy, postpartum care, parenting and family well-being.
- Ability to communicate respectfully and effectively with patients, families, community partners and healthcare teams.
- Strong relationship-building, organization, documentation, follow-up and problem-solving skills.
- Ability to provide patient-centered support while maintaining confidentiality and professional boundaries.
- Comfort using electronic health records, databases, Microsoft Office and other standard business technology.
TRAVEL AND SCHEDULE REQUIREMENTS:
- Ability to travel locally to community sites and events as required.
- Ability to work a flexible schedule when outreach events or program needs require evening or weekend coverage.

Pay rate: $24.6760–$30.7500 per hour
