Catholic Charities of Baltimore, Senior Communities is seeking a Service Coordinator to support residents stability, independence, and ability to age in place within an affordable housing community. The role connects residents with property management, service providers, and community partners while ensuring responsive coordination, accurate records, and compliance with program standards. Service Coordinator II and Senior positions also provide leadership for assigned staff, programs, or partnerships.
Catholic Charities Senior Communities develops and operates affordable, supportive communities for older adults with a resolve to nurture a spirit of purpose, wellness, and harmony among both our residents and colleagues. We offer 24 locations in Anne Arundel, Baltimore, Garrett County and Harford Counties and Baltimore City.
The work schedule is Monday – Friday 8:00 AM - 4 PM
This position will work between three locations:
- Tuesdays and Thursday at Everall Gardens located at 6100 Everall Avenue, Overlea, MD 21206
- Mondays and Wednesdays at Kessler Park located at 4230 Hollins Ferry Road, Lansdowne, MD 21227
- Fridays at St. Mark's located at 19 Winters Lane, Catonsville, MD 21228
JOB DUTIES & RESPONSIBILITIES:
- Develops personalized service plans that identify residents needs, connect them to appropriate resources, and support their overall well-being.
- Builds collaborative relationships with community partners to expand access to services, support resident integration, and improve outcomes for those
- Promotes resident engagement, social connection, and self-advocacy to support well-being and create a more supportive living
- Responds promptly to resident needs and safety concerns, coordinating appropriate supports and interventions to address immediate issues and long-term care planning.
- Supports positive communication during resident conflicts by helping all parties feel heard, respected, and focused on resolution.
- Assists residents, caregivers, family members, colleagues, and community members by providing support and resource information as needed.
- Documents daily all resident communications and service coordination activities accurately to support continuity of care, accountability, and effective follow-up.
- Connects residents with essential resources and information to support informed decision-making, independence, and community engagement.
- Supports a strong volunteer program that increases community involvement, strengthens support networks, and helps meet resident and community
- Promotes resident awareness and engagement, empowering residents to maintain independence, well-being, and safety in their homes.
- Completes required training and applies HUD guidelines and standards to support residents and maintain
- Coordinates with community partners to connect residents with care, services, and supports that promote well-being and quality of life.
- Collaborates with Property Manager(s) and other team members to serve residents and identify residents who may need assistance.
- Performs other duties as assigned.
POSITION SPECIFIC DUTIES:
Employees working in the following program(s) or position(s) have responsibilities that are in addition to the above core duties and responsibilities. These include, but are not limited to:
Senior Service Coordinator (207100) - Basilica Place and DePaul & St. Joachim House)
- Strengthens collaboration between the Agency and hospital partners to identify resident healthcare needs, coordinate access to appropriate resources, and strengthen continuity of care.
- Guides residents and families through rehabilitation, assisted living, and adult day care options by providing clear information, appropriate referrals, and timely follow-up that supports informed care decisions.
- Manages the internship program, including coordination of meaningful learning experiences and oversight of intern contributions, to expand service capacity for aging residents.
- Represents the Agency at hospital-sponsored community council meetings, contributes resident and organizational perspectives, and applies relevant information to strengthen community partnerships and resident support.
- Coordinates the Community Farmers Market, including partner and operational planning, to improve access to nutritious food, encourage resident participation, and strengthen community connections.
- Coordinates access to convenient, secure on-site banking resources and related information so residents can address routine financial needs safely and independently.
Service Coordinator (II and Senior)
- Manages and directs the work of one to three employees, including recruiting new hires, onboarding new employees, conducting performance reviews, and supporting accountability and effective team performance.
EDUCATION & EXPERIENCE REQUIREMENTS:
- Bachelors degree in social work, human services, gerontology, psychology, sociology, or a related field.
- Two years experience providing social services or service coordination, including assessing individual needs, developing service plans, connecting individuals with community resources and public benefits, maintaining accurate documentation, and collaborating with families and service providers.
- Demonstrated working knowledge of local services for the elderly and people with disabilities.
- An equivalent combination of relevant education, training, and experience that demonstrates the ability to perform the essential functions of the position may be considered.

- Strong verbal and written communication skills, including active listening and the ability to convey information clearly and respectfully to residents, families, colleagues, and community partners.
- Service-oriented and responsive, with empathy, patience, and sound judgment needed to understand concerns and support residents in identifying appropriate solutions and resources.
- Effective conflict-resolution and problem-solving skills, with the ability to remain calm, consider differing perspectives, and support respectful, practical resolutions.
- Professional discretion, integrity, and respect for confidentiality when handling sensitive resident, family, and organizational information.
- Strong planning and organizational skills, with careful attention to detail and the ability to prioritize multiple needs, maintain accurate information, and follow through in a timely manner.
- Collaborative and relationship-focused, with the ability to build trust, engage others, and influence coordinated action among residents, families, colleagues, and community partners.
- Requires proficiency in using computer systems and software to perform job function, including but not limited to, basic Windows PC, web browsing (i.e., Chrome, Internet Explorer, etc.), and Microsoft Outlook. Knowledge of other Microsoft Office applications, such as Word, Excel PowerPoint, Teams, and OneDrive is preferred.

- No remote or hybrid work availability based on core and essential functions of the position.
- Adjusting or moving objects up to 50# in all directions, which may include lifting, carrying, pushing, pulling, or repositioning items as necessary to perform job duties.
